2014-2015 Parent Tax
Verification Worksheet
(Version A)
5PTVWA
We need to verify your parent(s) 2013 income information reported on your FAFSA. If taxes were filed, the best way to verify this is by
using the IRS Data Retrieval Tool (IRS DRT) that is part of FAFSA on the web. (Follow instructions on next page). In most cases, no
further documentation is needed to verify 2013 income information that was transferred into the student’s FAFSA using the IRS DRT if
that information was not changed.
What you should do:
1.
Complete and sign this worksheet with your parent (s). Do not leave any answers blank. If blanks occur, the form may need to be
returned causing delays. The verification process can take 6 weeks or more to complete. Missing documents and incomplete
paperwork will cause delays.
2.
Submit this worksheet to the Financial Aid Office within 30 days. Documents can be submitted by:
Fax: (847) 543-3062

If your legal parents (your biological and/or adoptive parents) are married to each other, include both of them in the household, regardless of

are of the same or opposite sex.
If your parent is widowed or was never married, include only that parent in the household.
If your parents are divorced or separated and don’t live together, only report the parent with whom you lived more during the past 12 months.
If you lived the same amount of time with each divorced or separated parent, report the parent who provided more financial support during the
past 12 months or during the most recent 12 months that you actually received support from a parent.
If your divorced parents live together, include both of them in the household.
If your separated parents live together, include both of them in the household.
If you have a stepparent who is married to the legal parent whose information you’re reporting, you must include the stepparent in the
