$50 deductible per member per calendar year applies to Comprehensive Plan for regular and restorative care and
special restorative care and prosthetics.
Comprehensive Plan Annual Maximum
$1100 annual maximum payment per calendar year benefit applies to the Comprehensive Plan. $200 of this annual
maximum may be applied to services received outside of the service area or for use of a non-network dentist.

IMPORTANT ENROLLEE INFORMATION
The following information is provided in accordance with Minnesota Department of Health regulations,
which regulate HMO benefits in our fully-insured health plans.
ENROLLEE BILL OF RIGHTS
ENROLLEE INFORMATION
(1) Enrollees have the right to available and accessible
services including emergency services, as defined in your
contract, 24 hours a day and seven days a week.
(1) COVERED SERVICES: Services provided by the
plan will be covered only if services are provided by
participating providers or authorized by those
providers, except for emergency care or urgently
needed care or as provided under the travel benefit.
Your contract fully defines which services are covered
and describes procedures you must follow to obtain
coverage.
(2) Enrollees have the right to be informed of health
problems, and to receive information regarding
treatment alternatives and risks which is sufficient to
assure informed choice.
(3) Enrollees have the right to refuse treatment, and the
right to privacy of medical and financial records
maintained by the health maintenance organization
and its health care providers, in accordance with
existing law.
(4) Enrollees have the right to file a grievance with the
health maintenance organization and the
Commissioner of Health and the right to initiate a
legal proceeding when experiencing a problem with
the health maintenance organization or its health care
providers.
(5) Enrollees have the right to a grace period of 31 days for
the payment of each premium for an individual health
maintenance contract falling due after the first
premium during which period the contract shall
continue in force.
(6) Enrollees have the right to cancel an individual
contract and receive full premium refund if
cancellation is made within 10 days of receiving the
contract. However, any claims incurred by a enrollee
prior to cancellation will be the enrollee’s
responsibility.
(7) Medicare enrollees have the right to voluntarily
disenroll from the health maintenance organization
and the right not to be requested or encouraged to
disenroll except in circumstances specified in federal
law.
(8) Medicare enrollees have the right to a clear
description of nursing home and home care benefits
covered by the plan.
(2) PROVIDERS: Enrolling in this plan does not
guarantee services by a particular provider on the list
of providers. When a provider is no longer part of the
plan’s network, you must choose among remaining
network providers.
(3) REFERRALS: Certain services are covered only
upon referral. Please see your contract for referral
requirements. All referrals to providers outside the
participating network and certain types of health care
providers within the network must be authorized by a
participating network provider.
(4) EMERGENCY SERVICES: Emergency services
from providers who are not affiliated with the health
plan will be covered only if proper procedures are
followed. Your contract explains the procedures,
benefits and limitations associated with emergency
care from plan providers and non-plan providers.
(5) EXCLUSIONS: Certain services or medical supplies
are not covered. Please read your contract for a
detailed explanation of all exclusions.
(6) CANCELLATION: Your coverage may be canceled
by you or by this plan only under certain conditions.
Your contract describes all reasons for cancellation of
coverage.

appearance of your teeth, including tooth whitening
and veneers.
• Hospitalization or other facility charges.
• Local anesthesia or use of electronic analgesic billed as
a separate procedure is not covered. Nitrous oxide is
not covered. General anesthesia is not covered except
when dentally necessary provided by the attending
dentist in a dental office setting and required to
perform a covered oral surgery procedure.
• Orthodontic services are not covered.
• Orthognathic surgery (surgery to reposition the jaws) is
not covered.
• Services which are elective, investigative, experimental
or not otherwise clinically accepted, are not covered.
• Procedures, appliances or restorations that are
necessary to alter, restore or maintain occlusion are not
covered, including but not limited to: increasing
vertical dimension, replacing or stabilizing tooth
structure lost by attrition or erosion or realigning teeth.
Mandibular orthopedic appliances and bite planes are
also not covered.
• Services for the replacement of any missing, lost or
stolen dental prosthetics are not covered.
• Dental services related to certain specific programs of
treatment for which the date of service is prior to the

date the member became eligible to receive dental
services under this Contract are not covered.
Specifically, services related to a prosthetic or special
restorative appliance which was ordered while the
member was not covered under this Contract, or while
the member was covered, but installed or delivered
more than 60 days after termination of coverage, are
not covered.
• Diagnostic testing that is performed and billed as a
separate procedure is not covered.
• Dental services, supplies and devices not expressly
covered as a benefit under this Contract are not
covered.
• Prescription drugs and medications prescribed by a
dentist are not covered.
• There is no coverage for the placement, maintenance,
removal or treatment of complications associated with
dental implants. The dental restorations or prosthetic
appliances retained by dental implants are covered at
the prosthetic benefit level. Benefits are limited to the
amount that would be paid towards the placement of a
standard removable dental prosthetic appliance that
could be used in the absence of implant treatment. The
benefit will be paid when the restoration or prosthetic
appliance is finally placed. The implant placement and
restorative treatment must occur while the enrollee is
covered by this plan.
• Services provided to the member for which the
member is not required to pay are not covered.
• The portion of a charge for an otherwise eligible
service, which is in excess of the usual and customary
charge is not covered.
• Services for injury or illness either (a) arising out of an
injury in the course of employment and subject to
worker’s compensation or similar law; or (b) for which
benefits are payable without regard to fault, under
coverage statutorily required to be contained in any
motor vehicle or other liability insurance policy or
equivalent self-insurance; or (c) for which benefits are
payable under another policy of accident and health

insurance, Medicare or any other governmental
program are not covered.
• Except where expressly addressed in the dental
services shown above, when multiple, acceptable
treatment options exist related to a specific dental
problem, HealthPartners will provide benefits based
upon the least costly alternative treatment.
• Periodontal splinting is not covered.
• Charges for infection control, sterilization and waste
disposal.
• Consultations are not covered.
• Services or supplies resulting from dental diseases,
defect or injury due to an act of war, declared or
undeclared, are not covered.
• Services covered under the patient’s medical plan.
• Additional charges for missed appointments, office visit
appointments that occur after regularly scheduled hours
or appointments cancelled on short notice are not
covered.
• Onlays, veneers or partial crowns fabricated from
extraorally cured composite resin or porcelain.
Choosing a clinic does not guarantee that you will be seen by a specific dentist on the list. If you are
interested in receiving care from a specific dentist listed, please contact the clinic and determine if the
dentist is currently accepting new patients. Access to services does not guarantee access to a

HealthPartners has several different kinds of reimbursement arrangements with its providers.
Our goal in reimbursing providers is to provide
affordable care for our members while
encouraging best care practices and rewarding
providers for meeting the needs of our members.
Several different types of reimbursement
arrangements are used with providers. All are
designed to achieve that goal.
Some providers are paid on a "fee-for-service"
basis, which means that the health plan pays the
provider a certain set amount that corresponds to
each type of service furnished by the provider.
Some providers are paid on a "discount" basis,
which means that when a provider sends us a bill,
we have negotiated a reduced rate on behalf of
our members. We pay a predetermined
percentage of the total bill for services.
Some providers are paid a salary, with a possible
additional payment made based on performance
criteria, such as quality of care and patient
satisfaction measures.
We pay some groups of providers on a
"capitated" basis, which means that the provider
group receives a set fee each month for each
member enrolled in the provider group’s clinic,
regardless of how many or what type of services
the member actually receives. Provider groups
are, therefore, required to manage the budget for
their entire patient panel appropriately.
Occasionally our reimbursement arrangements
with providers include some combination of the
methods described above. For example, we may
capitate a provider for certain types of care and
pay that same provider on a fee-for-service basis
for other types of care. In addition, although we
may pay a provider such as a clinic using one
type of reimbursement method, that clinic may
pay its employed providers using another
reimbursement method.
Check with your individual provider if you wish
to know the basis on which he or she is paid.
Our approach to protecting personal information
As a health plan, we comply with federal and state laws regarding the confidentiality of medical records
and personal information about our members and former members. We’ve developed policies and
procedures to ensure that the collection, use and disclosure of such information complies with the law.
Whenever necessary, we obtain consent from our patients, or an approved patient representative when the
patient is unable to give consent, for disclosure of personal information. We give members access to their
own information consistent with applicable law and standards. Our policies and practices are designed to
facilitate appropriate and effective use of information, internally and externally, to enable us to serve our
members and improve the health of our members, our patients and the community, while being sensitive
