Lake County Community Health Improvement Partnership (CHIP) 700 South J Street Lakeview, OR 97630 7/22/2014 CAC/CHIP Physical Activity Workgroup Meeting Minutes Date and Time: Place: 7/22/2014; 3:00pm – 4:30pm Lake District Hospital, Penn Wilbur South meeting room Partnership members, staff, and guests in attendance: Jamie Davis, Jennifer Stephens, David Kerr, LuAnn Anderson, and John Adams AGENDA CAC/CHIP Physical Activity Workgroup 7/22/2014 / 3:00pm / Lake District Hospital, Penn Wilbur South Conference Room I Call to Order 3:00pm – II Welcome and Introductions 3:05pm – III Business 3:10pm – John Adams provided a review of the agenda. Guest Kathy Hayden presented on the Living Well with Chronic Conditions and Diabetes Programs. Training for the Living Well program runs September 912th in Hermiston. Mileage, food, and rooms are reimbursed. The program requires 2 volunteer leaders to run the program. The program is six 2 ½ hour volunteer led sessions. Kathy has grant funding for outreach and advertising for the program. Participants do not have to have a chronic illness necessarily; they can have a health obstacle that they want to improve on. Jamie Davis reported low turnout/interest when the program was offered through the OSU Extension Office. IV CHIP Strategies Update 3:40pm -John Adams provided updates related to the CHIP strategies. Lake County Mental Health received Transformation Grant funding for the Mental Health First Aid program for $38K, and they did not receive funding for the mental health integration with primary care. Additionally, the Physical Activity Scholarship program and the hospital’s Community Health Worker program all received Transformation Grant funding. -Oral Health: John reported that all the Superintendents in the county agreed to support the Dental Sealant and Fluoride Varnish programs at the schools in the fall. CHIP will work with Advantage Dental to implement the fluoride varnish in the schools and the dental sealant program in the spring, Lake County CHIP Coordinator: John V. Adams Ofc. They recommend pursuing an Oregon Community Foundation Grant for the dental sealant program and recommends that a community partner such as Lake Health District, the County, or the schools be the lead applicant to show strong community support. The LOI is due in September. John is working with Advantage Dental to develop the one pager and mail survey. -Physical Activity: John reported that the one pager is complete. The webpage address will be included on the physical activity promotional one pager. The webpage address is live; however, it is a work in progress. His plan is to disseminate the oral health and physical activity one pagers, together, beginning at the end of the month. IV Strategic Planning 4:00pm – 4 workgroup members participated in a voting process using turning point technology tools. Here are the results: The group prioritized physical activity health data 1. Data pointing toward the need for parks and recreation (2 out of 4) 2. Data pointing toward the need for health education, promotion, and outreach (2 out of 4) 3. Data pointing toward the need for physical activity for seniors (2 out of 4) 4 out of 4 believed that, based on the about data prioritization, that we do not need to develop another problem statement 4 out of 4 believed that, based on the about data prioritization, we do not need to develop new strategies; however, following discussion, the group agreed that we need new strategies within existing priorities The group did not prioritize new strategies 2 out of 4 believe that we do not need to research additional strategies VII Next meeting/Adjourn Goals and objectives 4:30pm – Lake County CHIP Coordinator: John V.
