NM Providers – GER- Step by Step Instructions DOH DDSD – Office of Systems Improvement Process Action Creating a GER The Step by Step Instructions for New Mexico Provider users with the General Event Report (GER) Submit role will outline the process on how to create and submit GERs for approval using the Therap system. How to Create a GER The General Event Report (GER) module is where providers report any incidents of an injury, medication error, out of home placement, unplanned use of ER/urgent care/EMT, etc. Revised 2/17/2015 To begin, ensure the ‘Dashboard’ page is displayed. Click on the ‘Individual tab’ on the left side of the Dashboard page. Under the ‘Care’ section, click on the ‘New’ link beside General Event Reports (GER). Select the appropriate program from the list (if you have access privileges to more than one program). This will open a new GER form. In the ‘Profile Information’ section, select the ‘Report Date’ for the incident. Other fields are auto populated. Page 2 NM Providers – GER- Step by Step Instructions DOH DDSD – Office of Systems Improvement In the ‘Event Date’ field use the calendar button to select the date when the event occurred (it defaults to the current date). If the event occurred anywhere outside of the agency’s physical location, choose the appropriate location from the drop-down menu in the ‘If not at responsible program’ field. Add a description of what happened before the event. You can describe anything that was going on before or led up to the event. For example if Mary fell and hurt her knee, but right before that she had been skipping down the hall, and you had verbally cued her to be careful, that’s what you could note in this box. Basically any antecedents or precursors to help you more fully describe the scene and what was going on leading up to the event, if you know. Complete the ‘Location Address’ section (if on site, you can check the box for ‘Same as program address’ and it will auto-fill). This will open another window with a more specific incident information form for each event type: Injury Medication Error Restraint Related to Behavior Restraint Other Death Other In the ‘Event Information’ window, add details pertaining to the event. Complete all required fields marked with a red asterisk (*). If the 'Other' option is selected in any of the questions, please specify details in the ‘If Other’ field. Use the body diagram to select specific body part(s) affected by an injury. Provide as much detail in the ‘Injury Summary’ section as possible. When complete, click on the 'Add' button at the bottom of the form, this will add the injury information to the GER form. Click on the 'Add Notification Info' button. This will open a pop-up window for adding notification information. In the ‘Notification Information’ page, fill out the notification details and click on the 'Add' button at the bottom of the form. Completing Other Sections Review/Follow up Comments This section can be used by supervisors, incident coordinators, agency directors, case managers and others having the required privileges to further document anything related to a given event. Revised 2/17/2015 Page 7 NM Providers – GER- Step by Step Instructions DOH DDSD – Office of Systems Improvement Submitting the GER Revised 2/17/2015 Files may be added in the ‘External Attachment(s)’ section, to the GER form. Click on the 'Add' button to add a file saved in your computer or click on the 'Scan' button to use scanner to add a document. Click on the 'Submit' button to submit the GER for approval. The report is given a 'Pending Approval' status and is put on the GER Approve List for users with the ‘GER Approve’ role. The GER form may also be saved to be completed later. Click on the 'Save' button to save it. The GER form will be found with an In Prep status in the GER ‘WorkList’ in the ‘General Event Report (GER)’ section of the Dashboard. Once submitted, the GER will no longer appear in the GER ‘WorkList’. Page 8
