TEACHING PRACTICE
Welcome to our practice. Our aim is to give you the best possible service.
This practice carries out teaching of 5th year medical students from
Kings. You may be asked if you are willing to see a student alongside
a GP
STATEMENT OF FITNESS FOR WORK
(Formerly sick certificate) You must consult with a doctor if you
want a medical statement, and they are only issued on the 8 th day of
sickness.
CONFIDENTIALITY
The practice follows agreed guidelines with regard to confidentiality.
The leaflet “How we use your health records” is available at
reception.
SEEING YOUR NOTES
To see your notes, ask at reception for a form to book an
appointment with the Secretary. A charge will be made for copies
of notes.

You can come to the surgery throughout these opening hours except for a
monthly closure on the third Thursday of the month between 12.45-4pm.
Core opening hours are 08:00-18:30 Monday –Friday. Extended hours:
Monday 18:30-20:00 and Thursday 18:30 – 19:00. Service during
extended hours is by appointment only

APPOINTMENTS
You can book appointments up to 2 weeks in advance.
To book an appointment you can use our online services (see Patient
Access below) or telephone and speak with our receptionists.
 All weekday appointments can now be booked in advance.
 Please tell the receptionist if you wish to see a particular doctor.
 Our normal doctor appointments are 10 minutes
 Please note that patients will not always be able to see their preferred
GP as they may not hold surgeries every day or they may be absent
from the practice e.g. on annual leave.
 We send text reminders to our patients regarding their appointments.
Please ensure that you provide us with your current number.
 Please book a separate appointment for each member of the family
who needs to see a Doctor, Nurse or Health Care Assistant (HCA)
Please let us know if you cannot come so that someone else can use the
appointment.
For emergencies on the day you will be booked in with the duty doctor
for a telephone triage call and the doctor will call you back. The doctor
will call you in if they think they need to see you.
Sometimes a delay is unavoidable, but we will try and ensure that you
are seen as close to your appointment as possible

Insurance and employment medicals, which are non-NHS and
for which a fee is charged, are arranged by special appointment via the
receptionists. Please also check the fee with them.
PATIENT ACCESS
Patient Access lets you use our online services. These include arranging
appointments, repeat medication, viewing part of your medical record
and updating your details.
You will need to bring in photo id to reception in order to use this
service. This is for data protection reasons. Please visit our website for
the link.
SERVICES
The doctors only take NHS patients. We provide a full range of
services including family planning advice, child surveillance, cervical
smears etc. The doctor will refer you to a consultant or for
investigations as appropriate. You will also need a referral for the
osteopath, counsellor or psychologist at the practice.

The nurses are available for BP checks, dressings, ear syringing and
other routine checks as well as travel immunisation and advice
(including yellow fever), health checks and smears. They can also
offer chronic disease management advice – asthma, diabetes, heart
disease and smoking cessation. Childhood immunisations may now
be booked on Monday 14.00-15.30 or with a nurse on other days.

The receptionists will help you wherever possible, give you any
further information you require about the practice, book
appointments and organise repeat prescriptions. They may be able
to give you results of any tests.

Please ring for results and other general queries after 2pm as the
mornings can be very busy.
REGISTERING
The baby clinic is run by Dr Katy Cameron with a nurse, health
visitor, nursery nurse and support worker on Thursdays, 1:30-3pm.
The clinic is for well babies only – child immunisations etc. If your
baby is ill, please consult the doctor routinely. Please refer to our
website for changes to the clinic dates.
The health visitor works from Townley Road clinic. Telephone
numbers can be obtained from Reception.
Dr Addo runs a minor surgery session on Wednesdays on referral.
 The osteopath attends on Fridays.
Free condoms and pregnancy testing are available from the surgery.
Patients are able to register as long as you are living within our
catchment area. There is a map on our website.
You can register online via Patient Access (please see Patient Access
above) or in person (after 09.30 am and 30 minutes prior to closing).

Please can you let us know if there are any changes to your name,
address or telephone number so we can update your records.
ACCESS FOR DISABLED PEOPLE
There is a disabled parking bay controlled by permit, disabled toilet
and ramped access to the surgery. Please tell reception if you have
difficulties with stairs so we can arrange to see you downstairs.
COMPLAINTS
If you have a complaint you may put this in writing to the Practice
Manager who will look into your complaint. We will aim to:
Investigate the problem with all members of staff concerned
Notify you of the outcome of the investigation with a satisfactory
explanation.
Identify the problem with a view to improving the service we offer
to our patients.
Please ask for a copy of our complaints policy at reception. We
would like to resolve the problem and have the opportunity to
improve our service to you, but if you choose not to contact us you
can approach NHS England direct at [email protected] or
on 0300 311 22 33.
If neither body helps resolve the issue, you have the right to
approach the Ombudsman on 0345 015 4033 and at
[email protected]
Clinical Commissioning Group (CCG)
This Practice is located in Southwark and works with NHS Southwark
Clinical Commissioning Group (CCG). The CCG is made up of all
44 GP surgeries in the local area. To find out more about them

REPEAT PRESCRIPTIONS
You can request your repeat prescription in the following ways:
In writing the practice has a repeat prescription request form.
By using our online services (please see Patient Access above)
Requesting from your nominated pharmacy
Please note:
We only take telephone requests from housebound patients. This is for
reasons of patient safety.
Repeat prescriptions process requires two working days so please request
your medication a few days before they run out.
Tell us which pharmacy you would like the script to be sent to and we will
send it across electronically. Occasionally we may ask you to come in for a
review. This is usually for safety reasons.
EMERGENCY CALLS & OUT OF HOURS
If you need the doctor when the surgery is closed, call SELDOC on 020
8693 9006. In a medical emergency dial 999. If you are not sure, you
may also phone NHS Direct on 0845 4647 or go to www.nhsdirect.nhs.uk
for advice and guidance.
HOME VISITS
If you are too unwell to visit the surgery and feel that you need a home
visit please ring the surgery as early as possible to speak to the Duty Dr.
They will decide whether a visit is necessary after speaking to you.
