care practitioner (PCP) is at the core of our
patient-centered medical home (PCMH)
approach to healthcare. In this model,
your PCP doesn’t just care for your dayto-day health needs and treat you when
you’re sick; he or she also serves as your
healthcare advocate by coordinating all
aspects of your care throughout our system.
Your PCP’s first priority is to keep you well
by providing preventive care and helping
you to manage your own health, thereby
improving your quality of life and reducing
your healthcare costs.
But as the “hub” of your healthcare
universe, your primary care team is also
your direct connection to all the other
integrated care services available through
Day Kimball Healthcare. If specialized care
becomes necessary, your PCP is there to
not only provide referrals but also to ensure
that all of your care providers are working
together effectively and efficiently to
deliver the best possible care.

Walk-In Center
Your primary care practitioner (PCP) is the hub of your healthcare
universe, and is there for you when you’re suffering from a minor illness
or injury with same-day appointments during regular office hours. But
what if you suffer a sudden illness or injury when your doctor’s office is
closed? When you can’t see your PCP (or if you don’t have one), we’re
still here for you at our Walk-In Center at the Day Kimball Healthcare
Center in Plainfield.
Walk-in care is the delivery of urgent medical care for injuries or
illnesses that require immediate attention but are not life-threatening
or serious enough to warrant a visit to an emergency department. With
evening and weekend hours to support your PCP, our Walk-In Center
provides a convenient, more efficient and lower-cost alternative to
the emergency department for minor illnesses and injuries, without
sacrificing quality of care. The center is staffed by highly-qualified
doctors, physician assistants and nurse practitioners.
Walk-In services include treatment for
• minor cuts, lacerations, bruises and fractures
• colds, fever and viral illnesses
• rashes
• ear infections
• swollen glands and sore throats
• respiratory infections such as bronchitis
• minor abdominal pain and gastrointestinal infections
• sports or work related injuries
Our Integrated Network of Care
Our integrated system allows your Day Kimball Medical Group PCP to
“talk” about your health with our Walk-In Center staff and vice-versa,
through electronic medical records that provide instant access to your
medical information. Our specialists, diagnostic imaging technicians,
laboratory technicians and other healthcare professionals utilize this
system as well, so your entire care team can make an informed and
comprehensive evaluation of your condition. In this way, you can
be assured of top-quality care within a truly integrated and holistic
approach that also eliminates errors and redundant services to help you
control your healthcare costs.
Once you’ve left the Walk-In Center, we’ll continue to support your care
and recovery by keeping your PCP informed of your visit. If you don’t
have a primary care provider, our staff will instruct you on how to easily
find one who is accepting patients. This will ensure that you’ll receive
quality follow-up treatment as well as important preventive healthcare
and other integrated services to help keep you well in the days ahead.
Billing and Payment Information
Most health insurance plans are accepted, and patients have lower
copay requirements than at the Emergency Department. Please check
with your insurance carrier about your specific coverage, copays and
deductibles. Services are also available on a self-pay basis. If you have
