Category Nomination Guidelines
The Innovation – Clinical Area Award will be given to an individual or team in a clinical area who demonstrates
an innovative way of thinking. The award recognises the efforts of a new or improved process, program or
initiate with a demonstrated improvement in quality, service, financial operations and/or safety. The results
must be measurable and show positive outcomes. Either an individual or a team may be nominated for this
award. A team is comprised of three or more team members, including the Team Leader and can be cross
functional or work-unit based, but within a clinical area. If nominating a team, one person on the team must
be designate as Team Leader.
Marking Criteria
Relevance
How the innovation addressed the need for the new or reviewed process
/program /initiative.
Effectiveness
How the outcomes related to quality, service, financial operations and/or safety
Innovation
How was creativity and innovation demonstrated
Transferability
Describe how this innovation can be transferred, implemented or applied to
other departments group wide.
Requirements and Eligibility
 The nominee must be part-time or full-time, and actively employed with Bon Secours for at least six
months on or before the closing date for nomination.
 Volunteers are not eligible for this award.
 Nominees can be any grade/level of staff.
 Nominees must not have any performance/disciplinary issues within the past year.
 Nominees must exemplify the Bon Secours mission/vision statements and core values.
 The nomination must be endorsed by the hospital manager
Submission Process and Instructions
 The form must be typed – we cannot accept handwritten forms.
 Completed forms should be uploaded on the online nomination page.
 Optional Supplementary materials should be included in the appendix and limited to three pages.
 The deadline for nominations is 6pm, March 21st 2016.
Application Review Process
All applications will be reviewed by a Judging Panel, which includes Bon Secours Health System Executive and
Non-Executive members and leaders from outside the organization. The shortlist will be announced on the
week of the 11th April and the event will take place on May 6th. Any questions regarding the application
