The Cleveland State University Choose Ohio First Undergraduate Adolescent/Young Adults (Secondary) Pre-Service
Scholarships will provide up to $4,700 a year in renewable scholarships for students who are accepted into teaching
licensure programs (minor in education).
Required Eligibility and Requirements for D-STEM Choose Ohio First Scholarship
Participants must:








Be enrolled in degree program in the College of Science in mathematics and/or science.
Participate in all activities, trips, meetings and events sponsored by Choose Ohio First teacher preparation
program and make progress toward an academic degree.
Maintain a minimum 2.5 grade-point-average.
Be United States citizens or permanent resident.
Be an Ohio resident.
Have a current FAFSA on file at Cleveland State University.
Not be a recipient of Honors, Athletic, or any other FULL tuition scholarships (recipients of PARTIAL Honors,
Athletic or other scholarships are eligible).
Be registered for all their classes by the application deadline.
Other Requirements:

Letter of recommendation from past or current math or science teacher and attachment A form
Transcripts-Unofficial transcripts may be used
Statement of Goals (see attached)
Benefits:


Earn a Choose Ohio First Scholarship of up to $ 4,700 a year. This scholarship is automatically renewed for up to
two years depending on the life of the grant if participants meet all of the requirements.
Upon graduation earn an Ohio license to teach in your chosen area. Science and math teachers are among the
most sought after teachers.
Information about this Scholarship:





The maximum award is half the cost of tuition per semester, up to $4,700 total for the fall and spring semesters.
It is not a need-based scholarship, but a current FAFSA is required for processing.
This scholarship is applicable to tuition only and if you have taken out loans to pay for your tuition, your loan
amount will be reduced by the amount of your scholarship award.
You can be a student worker at CSU and receive this scholarship.
CSU faculty or staff members and their family members who are eligible for other tuition credits and benefits are
not eligible for this scholarship.

D-STEM Letter of Recommendation and Form (Attachment A)
One letter of recommendation must be provided. This letter should come from a science and mathematics
teacher who is familiar with your ability in mathematics and science. A form must accompany the
recommendation letter.
Statement of Goals
Please describe why you are interested in the CSU D-STEM Choose Ohio First Scholarship. Your essay
should discuss your educational experiences, interest in science or mathematics, and what aspects of teaching
you are most interested in or would like to learn more about. Describe how participating in this program will
help you to reach your long-term career goals. (maximum of 400 words)
