Roofnado™ AnchorDeck™, StormTread™ and ShingleLock™ VIP Program Terms and Conditions:
A minimum of 30 rolls of ShingleLock™ or StormTread™ or 98 rolls of AnchorDeck™ must be purchased to be eligible for a $100 credit card. Offer is only valid for one credit card per “bill
to” location. To receive your credit card you must complete this application form and return via email (along with legible copies of invoices) to: [email protected]. All invoices must
clearly show the following information: (i) The type and number of rolls purchased; (ii) The invoices must be dated no sooner than June 1, 2017; (iii) The invoice amounts paid; and the
purchasing roofing contractor company name with full address and contact numbers. Credit cards will be mailed out to all eligible participants in quarterly instalments. Invoices can be
submitted throughout the duration of this program. This offer is void where restricted or prohibited by law. These Terms and Conditions are subject to change without notice at our sole
discretion. Amendments to the Roofnado™ VIP Program will be posted to our website at: www.roofnado.com/vip. Participant authorizes random verification of invoices submitted. All
