CHILD CARE CENTRE
The personal information collected on this form is collected under the authority of the Social Care Facilities licensing Act and Daycare
Regulations, the Colleges Act and the Freedom of Information and Protection of Privacy Act of Alberta. It will be used by the Child Care Centre,
for the purpose of evaluating and improving our quality of service. The information will be protected in compliance with the provisions of the
Freedom of Information and Protection of Privacy Act of Alberta. The information will be disclosed to Alberta Child Care Accreditation Program
(ACCAP). This information will be retained for a period of three years at which time it will be destroyed in a secure manner. If you have any

The Day Care Centre at Red Deer College has been selected to participate in a pilot program for the Alberta Child
Care Accreditation Program (ACCAP). The purpose of accreditation is to ensure a high quality service to children
and families. We are currently in the process of looking closely at all aspects of our centre’s operation including
policies and procedures, activities, communication, facilities, and governance.
This survey asks for your feedback on various areas that will be addressed during the accreditation process. Please
take a few moments to complete the questionnaire. Your opinions will play an important role in helping us determine
what we are doing well and where we can improve.
