I. Procedure Title: Receipt and Sending of Documents via Facsimile

II. Overview/Procedure Description:
A. Policy: The Puerto Rico Clinical and Translational Research Consortium
(PRCTRC) will facilitate the delivery and receipt of documents containing private
information from participants.
B. Purpose: Ensuring the confidentiality and security during shipping and receipt of
private information of the participants as required for the law and/or regulatory
agencies.
III. Area(s) of Responsibility: Administrative staff, clinical staff, researchers and
collaborators.
IV. Procedure Details:
A. Any transmission of private information of subjects should take place at the fax
machine located in the nursing area Office 143 C.
B. The Principal Investigator or collaborators must provide this fax number to all
companies or entities that will send private health information using this source.
This information includes: laboratory results, ECG results, any consultation with
the sponsor of the study and any document that in any way divulge private health
information of the subjects.
C. This fax machine is to be use by research professionals and is exclusively
intended for the transmission of private health information. It is therefore
forbidden to receive or send private information of subjects through the fax
machine located in the administration area.
D. Administrative staff or non-research related personnel may not transmit or
receive documents through this machine.
