Susac Syndrome Cognitive Assessment (SAGE Test)---Susac-Sage Patient ID# Date________ (Month) ______ ______ (Day) (Year) The score for your SAGE test will be calculated by our research team The SAGE test is designed to identify cognitive impairment. It was developed by Dr. Scharre, a neurologist at Ohio State University. Dr. Scharre has enthusiastically given the International Collaborative Study of Susac’s Syndrome permission to use the SAGE test in our study. SAGE is a brief self-administered cognitive screening instrument to identify Mild Cognitive Impairment (MCI) and early dementia. Average time to complete the test is 10 to 15 minutes. There are four forms of the SAGE test. Only one test form should be given. It does not matter which form is taken, as they are all interchangeable. Click here to link directly to the tests.
