Elements of Clinic Cards
• Specific to Surgery
• 2-sided
• Requires written narrative feedback
• Requires comment on at least one,
but not more than four Entrustable
Professional Activities (EPAs)*
• Requires acknowledgement of direct
observation of Hx/Px
• Requires preceptor/supervisor
completion and signature

Completion of Clinic Cards
Students are encouraged to complete the
information outlined by the red oval.
• Each student, at a minimum will have one
Clinic Card completed per week (e.g., a
student assigned to wards).
• Each student, requires a Clinic Card for each
separate clinic to which he/she is assigned.

• For a given Clinic Card, a
supervisor/preceptor is required to
comment on at least one EPA, but no more
than 4 EPAs.
• Clinic Cards covering a week long period
would likely cover more EPAs than a Clinic
Card reporting on a half-day clinic.

Step 1:
• Select 1, 2, 3 or 4 EPAs for which
meaningful narrative coachingstyle comments will be provided
on the other side of the card

that the supervisor provide
assessment on a particular EPA.
The student will be monitoring
his/her assessments to ensure that
all required EPAs are being
assessed adequately.
Step 2:
EPA-1: Appropriately requested
and used an interpreter for non-English
speaking pt. Used pt centred approach.
EPA-5: Critical
information missing from notes, such that
clinical reasoning not apparent. Suggest
team-mate review notes with student.

to cover areas of strength and
areas for improvement.
• For a given Clinic Card, a
supervisor/preceptor is required to
comment on at least one EPA, but
no more than 4 EPAs.
• Complete other information
regarding witnessed Hx/Px

procedures (including casting) that
were directly observed by the
supervisor.
• Sign card acknowledging whether
supervisor is a resident or faculty
member.
