BASKETBALL PLAYER AGREEMENT
I agree to play for this team in accordance with the regulations of the Juneau Parks and Recreation
Department. In signing this agreement, I certify that I am eligible under the current Park and Recreation
League rules. I agree to pay a player fee upon signing the agreement as a condition of participation. This
fee is NON-REFUNDABLE NOR TRANSFERRABLE TO ANOTHER PERSON.
INJURY/PRECAUTION FACTS: I understand basketball is a game played within a confined space in an indoor
environment that requires skill and ability to run and jump and dribble a ball. Anyone playing basketball should be
aware of these conditions and the fact that a ball, playing surface, or another player may inflict injury. The force of
bodily contact or hit ball may cause severe bruising and/or broken skin, bones and /or teeth. A blow to the head
may cause a concussion and/or cranial fracture which may lead to a terminal injury as well as less severe injuries.
A ball striking an eye may cause irreparable damage or result in a total loss of vision. Because players are playing
in a confined area, misjudgment of distance may result in a collision with equipment or other players. Collisions
between two or more players may cause injury to all parts of the body including concussion, cranial fracture, broken
bones, teeth, eye injury, cuts, bruises and skin abrasion.
CATASTROPHIC INJURIES RELATED TO BASKETBALL INCLUDE BUT ARE NOT LIMITED TO:
1) Loss of vision as a result of collision with another person, equipment, ball and/or wall; 2) Fatal and/or brain injury
as a result of collision with another person, equipment, ball and/or playing surface; 3) Neck or spinal injury as a
result of collision with a person, wall, equipment, and/or ball.
SEVERE INJURIES RELATED TO BASKETBALL INCLUDE BUT ARE NOT LIMITED TO: 1) Broken bones,
broken teeth, concussion, severe bruises, tissue injury related to a collision with a person, partition, wall, ball, and/or
court surface or equipment; 2) Broken bones, broken teeth, severe bruises, damage to tendons, ligaments and
muscles as a result of falling or contact with another person.
EXEMPTION FROM LIABILITY: I recognize the activity, basketball, for which I am registering involves risk of
damage, loss and personal injury, and in consideration of your accepting this registration, I waive and release the
City and Borough of Juneau Parks and Recreation Department, its officers, agents, servants, employees, and
lessors from any and all liability, claims, demands or actions or causes of actions whatsoever arising out of any
damage, loss, personal injury suffered or sustained by me in connection with or arising out of or resulting from any
and all of the activities contemplated by this agreement, whether such loss, damage, or personal injury resulting
from the negligence of the City and Borough of Juneau or any of its agents, employees, servants, or representatives
from any claims brought against it by parties claimed by me to be responsible for any damage or loss that I incur.

2005 - 2006 ORDWAY BASKETBALL PACKET
Dear Manager,
The 2005 - 2006 Ordway Basketball season is scheduled to begin Sunday, November 13, 2005.
The season will be ten weeks long (not including breaks during holiday facility closures) and will
include 18 regular season games and a post-season tournament. Teams play an average of two
games per week.
Team registration will be held on Thursday, October 20 at 5:00 p.m. in the Assembly
Chambers, located on the main floor of the Municipal Building, 155 South Seward Street.
Enclosed in your packet are important items that you need to complete prior to registering your
team for the 2005 - 2006 season. Your packet should include the following items:

The name, address and signature of manager AND alternate manager must be filled
out on the Roster/Classification Sheet.

Completed player agreement for each participant.
Player agreements must be placed in the same order as they are listed on the team roster.
A minimum of seven player fees. Please do not attach or staple money to player
agreements.

We cannot accept two-party checks.
Your team registration will not be taken if any of the above items are omitted. You will need to

REGISTRATION - Thursday, October 20
Team registration at 5:00 p.m. in the Assembly Chambers, located on the main floor of the
Municipal Building, 155 South Seward Street. At this time teams may sign up for one preseason practice.
LATE REGISTRATION - Friday, October 21
Late registration, beginning at 9:00 a.m. and ending at 4:30 p.m., at the Parks and
Recreation office, located on the second floor of the Municipal Building. A $10.00 late fee
per team will be charged at this time. Teams shall be accepted on a space-available basis
only.
MANAGER'S MEETING - Thursday, November 10
5:15 p.m. in the Assembly Chambers. Schedules will be distributed at this time and rules
will be discussed. One representative from your team must be present to receive a win
which will count towards final standings.
PRE-SEASON PRACTICES - Saturday, November 5 - Sunday, November 6
LEAGUE PLAY BEGINS - Sunday, November 13

G E N E R A L I N F O R MA T I O N
TEAM CLASSIFICATION PROCEDURE The Parks and Recreation Program Supervisor
and the Player Advisory Board will review classification sheets to determine which division
each team will be placed in. Teams will be placed according to preference as indicated on
classification sheet whenever possible. There must be a minimum of four teams to form
a division.
Within 24 hours after teams have been assigned to divisions and day of play has been
determined, Parks and Recreation staff will contact each team manager and inform them of
their respective placement and day of play.
APPEAL PROCEDURE There will be a period of two business days (after notification) in
which managers may appeal to the Department of Parks and Recreation for a change of
divisions. This appeal must be made by the team manager. It must be in written form and
must list the reasons why the placement of your team is not appropriate. Please note that
the day that each division is assigned to play on will not be changed unless it creates a
conflict for three or more players on a majority of the teams in any division. Appeals will be
reviewed by the Program Supervisor and the Player Advisory Board. Appeals will be
answered in writing within two business days of being received.
Age Requirement Players must be 17 years of age as of the first scheduled game
of the Ordway League.
Multiple Team Sign Up Players may not register on more than one
Parks and Recreation basketball roster.
UNIFORMS
All teams are required to have numbered jerseys of the same color by their first
game. Numbers must be permanent and readable (preferably a minimum of six inches
high). Numbers constructed of tape or written with pen, pencil or permanent marker will not
be acceptable. Numbers need to be on one side of the shirts, preferably the back. Penalty:
Player will be ineligible for game participation. Teams must have numbered jerseys by the
first game or forfeit.
LOCATION AND GAME TIMES FOR ORDWAY LEAGUE
Games will be played at Marie Drake, Floyd Dryden, Gastineau Elementary and Dzantik'i
Heeni Middle school gyms.
Game times vary between 6:00 p.m. and 10:00 p.m. on weekdays and between 11:00 a.m.
and 7:00 p.m. on Sundays. The games will consist of two thirty-minute running clock
halves, with the last three minutes of the game being stopped clock (unless there is a 20
point lead in score).
Reschedules may occur during the season due to activities at the schools. Parks
and Recreation will inform managers of any "bumps" as soon as possible.
