NMC outcomes and performance criteria – Stage Two - LD Nursing
Part A - Professional Values
1. Display professional behaviour according to The Code: Professional standards of practice
and behaviour for nurses and midwives (NMC 2015)
2. Work with clients/patients in an ethical and equitable manner
3. Maintain appropriate confidentiality in all aspects of their work
4. Accept delegated tasks with due regard to client/service need and personal limitations
regarding role, knowledge and skill base
5. Take responsibility and accountability when delegating care to others demonstrating an
understanding of the roles undertaken by other members of the health and social care team
6. Work within the laws governing health and safety at work e.g. manual handling technique
and infection control
7. Accept feedback from clients/patients, mentors, practice supervisor and others in a
professional manner and reflects upon it to enhance their knowledge and skill base
8. Reflect upon their own practice discussing issues with appropriate individuals/team
members to enhance their own learning and formulate development plans
9. Actively manage personal stress through the appropriate supervision process
10. Demonstrate an understanding of the various roles undertaken by the learning disability
nurse to meet the diverse health needs of people who have learning disabilities
11. Act as a role model to others and ensure colleagues work within local policies
12. Identify the relevant theory that underpins their practice

Part B – Assessment
1. Utilise a range of augmentive communication strategies to promote the professional
therapeutic relationship and support individuals to communicate their needs
2. Review referrals with the practice supervisor to maintain safe practice and establish the
scope of the person centred work to be undertaken
3. Promote the rights and choices of individuals and encourage all forms of advocacy
4. Obtain consent prior to any intervention; and ensure that the right to withdraw consent is
respected and documented
5. Compile an holistic pen picture of the individual or group that reflects current health and
wellbeing issues
6. Undertake in collaboration with the individual or group a comprehensive personalised
assessment of need, acknowledging differences in health status, personal ability, age, beliefs
and cultural practices
7. Undertake risk assessments in relation to clients, patients, the environment, infection
control, self and others
8. Prioritise and document in collaboration with the individual and or their carers the relevant
information and outcomes from the assessment
9. Consider aspects of the persons medication in relation to holistic care
10. When appropriate undertake and document a range of baseline assessments including:
weight, height, temperature, pulse, respiration, blood pressure and urinalysis, recording and
responding appropriately to results outside the normal range

Part C - Planning Care
1. Construct under supervision appropriate care plans and prepare the clients/patients for the
delivery of care in keeping with ILP guidance and local and national standards
2. In collaboration with the clients/patients and or their carers and the mentor formulate and
agree realistic aims and outcomes for the intervention taking into account issues in relation
to transition
3. Establish a realistic time frame for the intervention, including evaluation dates
4. Select appropriate resources identifying the rationale underpinning the choice including
financial resources
5. Demonstrate an understanding of the evidence base supporting the intervention
6. Identify links with the wider health and social care team and carers demonstrating an
understanding of the role they play in the planning and delivery of holistic care
7. Monitor and record any concerns regarding the safety and well-being of children/vulnerable
adults in accordance with local policy and the law

Part D - Care Delivery
1. Demonstrate that consent is understood, maintained and reviewed during any intervention
2. Accurately record relevant information and actions within the care plan in keeping with ILP
guidelines and local and national policies
3. Problem solve to adapt the planned intervention to meet varying and unpredictable
circumstances as they arise
4. Dispose of equipment safely utilising the correct health and safety/ infection control policies.

Part E - Evaluation of Care
1. Review the interventions according to the ILP guidance and the agreed time scales,
identifying progress, completion or re-assessment
2. Effectively handover verbal and or written information related to the intervention
3. Upon completion of the intervention evaluate and record the outcome with the
clients/patients and or their carers
4. Disengage from the professional therapeutic relationship in an appropriately person centred
structured manner
5. Reflect upon practice within each client file demonstrating personal and professional
development
