INSTRUCTIONS
Go to the following website: http://friscoisd.rankonesport.com
Click on the tab that says Electronic Participation Forms
Start with the first form: FISD Travel Form (Required)
a. At the top of the page you need to fill out your students first name, last name, student ID, and
school they will be attending in 2016-2017
b. Please fill in the emergency contact information being sure to leave no blanks. If you must leave
something blank, simply fill in with N/A
c. Next, fill in your primary insurance information being sure to leave no blanks. If you have no
insurance, the fill in the blank with N/A
d. Please fill in the short answer fields about Allergies, Medications and Other Medical Conditions as
we use this information for medical alerts and drug testing purposes.
e. Over The Counter Medication Consent: please check this box if you authorize your athlete to have
basic medications in case of an injury, i.e. Tylenol, Advil
f. Type your athletes name in the next box giving them consent to participate in UIL sports.
g. Have your athlete type their name in the next box and sign the form using your mouse. Be sure
to check the I Agree box next to the signature.
h. You, the parent, need to type your name in the next box and sign the form using your mouse. Be
sure to check the I Agree box next to the signature.
i. Type your email in the box labeled Notification Email at the end of the page and then hit the
Submit button.
The second form, UIL/Athlete Participation Form, (Required)
a. Read the opening statement and then go to the first form labeled 1. FISD Parental Code of Ethics.
The form will open in a new window. Read the form, then close the window and check the box
next to the form name. Doing this means you agree to that particular form.
b. Open the rest of the forms 2 through 12 being sure to read each form and then checking the
corresponding box next to each form number.
c. After all forms have been read and the box checked next to the form numbers, fill in the next
section being sure to not leave a box blank. If you must leave a box blank then simply type in
N/A.
d. Have your athlete type their name in the next box and sign the form using your mouse. Be sure
to check the I Agree box next to the signature.
e. You, the parent, need to type your name in the next box and sign the form using your mouse. Be
sure to check the I Agree box next to the signature.
f. Type your email in the box labeled Notification Email at the end of the page and then hit the
Submit button.

Finally, please click on the Physical/Medical History tab.
a. Fill out the first page (Medical History) online as accurately as possible and complete the
checkboxes. Do not fill in the last line as this is for School Use Only.
b. On the second page (Physical form) only fill in the Student’s Name, Sex, Age, and Date of Birth.
c. Print out these two pages and manually sign the first page with a black pen and then take them
to your doctor of choice for a sport physical.
d. Return these forms to the Athletic Trainer or your student athlete’s Head Coach.

Athletic periods are:

Practices before school for
football begin at approx. 7am;
after school practices will end
at approx. 5:00pm.

Hair and Grooming
Hair Code (FISD policy)
– Hair must be clean, well groomed and out of
athlete’s eyes. It must be styled in a way not
distracting to others.
• No unnatural dying of hair or hair cuts with designs
• No dress that is unbecoming of a Cobb athlete
– NO jewelry in athletics.
• Game day dress
– Players will dress as a team.
Loyalty
• Cobb comes first!
– Club and outside activities should not come
before Cobb Athletics.
• Communication is the key.
– We will do our best to accommodate other
activities.
• Continued issues may result in removal
from the team.

Transportation
• We will provide transportation to and from
all games.
• Students in Football, Volleyball and
Basketball are required to stay and watch
their teammates play.
– Exceptions can be made with advance notification for
unique circumstances.
– Playing time taken for those that sign out with parents
after a contest without advance notice.
• 15 Minute Rule
Grades
• No pass. No Play.

– 3 week progress report.
• Dressing out and attendance at practice and
games is expected from every athlete…. even
if injured.
• Athletes must follow all athletic guideline's as
agreed to in our Athlete’s Promise.

Parent Expectations
• I will always keep in mind that it is a privilege for my child to
participate in a sport rather than a right.
• I will always model good sportsmanship at competitions by
the way I treat all athletes, coaches, officials and other fans.
• I will insist that my child always demonstrate good
sportsmanship and treat other athletes, coaches and officials
with respect.

grace by the way I act in each of those situations
• I will always teach my child the importance of
competing with integrity and will not help him or her
cheat in any manner.

athlete, I am representing my child’s team at

Parent Expectations
• If I have an issue with a coach, I will always approach him or
her in an appropriate manner, and should I have the need to
discuss a concern, then I will follow the process designated by
the athletic department and school administration.
• I will always refrain from coaching my child immediately
before, during and immediately after competitions because of
the potential negative effect it may have on my child’s
performance.
• I will always remember that I have had my opportunity in
athletics and this experience is about my child.
• I will have a basic understanding of the rules of my child’s
sport.

New Football Schedule
• We are speeding up the beginning of the
football season starting time.
– This will require your Athlete to be available for 1
Saturday practice (Labor Day Weekend: Sept. 3rd)
• If they miss any practices, they will be unable to
participate in the first scrimmage of the year.
Volunteers/FCA
• August 18th Football meeting and equipment
pick up.
• 5:00-7:00 pm (Meeting at 5:00)
• FCA always needs and wants parent
involvement….breakfast, tailgate, camp
sponsors etc…
