Recovery and Rehabilitation team Information for patients What is the Recovery and Rehabilitation team? The Recovery and Rehabilitation team is a group of professionals who work with people in the borough of Milton Keynes who have been and / or are currently experiencing a severe enduring mental health condition, such as non-organic psychosis, schizophrenia or bi-polar disorder. The Recovery and Rehabilitation team consists of psychiatric nurses, nurse prescribers, senior practitioners, psychiatrists, an occupational therapist, an assistant psychologist, an administrator and medical secretaries. We aim to: •To provide treatment for distressing experiences of severe enduring mental health problems •To provide a range of psychosocial interventions •To work with other agencies to support the person •To provide support in finding a way to live a richer, fuller and more meaningful life •To provide support in moving on from services. The Recovery and Rehabilitation team is co-located with the Adult Mental Health Social Care Team, the Assertive Outreach Team (AOT) and the Early Intervention in Psychosis Team (EIPT), enabling quick access if we feel your needs would be better met by another team. What is a psychotic experience? The term is used to describe conditions that affect the mind, where there has been some loss of contact with reality. Psychotic experiences vary a lot between individuals and everyone’s experience will be unique. Common signs might be: •Losing trust in people •Jumbled thoughts •Trouble sleeping •Loss of energy •Isolating self or withdrawal •Changes in mood •Hearing voices others cannot hear •Feeling suspicious of others •Unusual beliefs not shared by others •Believing others are against you •Saying things that do not make sense. These experiences seem so real that often the person does not realise that he or she is having psychotic experiences. Experiencing the above can be scary, confusing and distressing. The Team was developed to support you in finding a way to cope with symptoms and to lead a meaningful life. What does ‘recovery’ mean? Recovery is a very personal experience and is different for everyone. Recovery is a person’s individual journey of how to live well with symptoms or vulnerabilities, how to live a fulfilling and meaningful life. What ‘recovery’ means to any one person may even change over time. Therefore, we take a personcentred approach and support you in defining what recovery means for you and how you may live by those values. How to access our service You will need to be referred to our service, after having received a diagnosis of severe enduring mental health disorder. A referral is usually made by secondary mental health services, like Assessment and Short Term Intervention service, community mental health teams or the Campbell Centre mental health inpatient unit in Milton Keynes. What happens once you have been referred? All referrals are reviewed within the multidisciplinary team to see if we are the right service for you. If the referral is accepted by the Team, you will be assigned a clinician within the team. One of our team will call you to arrange a first appointment. Appointments can be at one of the health centres, the hospital, at your home or another quiet place where you can be asked about your difficulties, resources and circumstances without breaching your confidentiality. You can bring another person for support if that is your wish. What happens after the initial appointment? Your clinician will work with you and will discuss with you a plan of care based on your needs. A plan of care is a written document that details the areas with which you need support and how this support will be provided. This could be a yearly letter or a Care Programme Approach (CPA). The plan of care will be reviewed regularly and will be adapted to meet your needs as they change. You and your clinician will agree on how often you meet, how often your medication is monitored and what other support may need to be arranged. Plans may include smoking cessation classes if you wish to stop, anxiety management workshops, mindfulness or psychosocial groups. You will also be invited for routine physical health checks.
