Complete this application
Declared Major in a STEMM discipline
Cumulative GPA at least 3.0
Ohio Resident
High School diploma or acceptable equivalent
Submit the Choose Ohio First Scholarship Application and all requirements/ attachments (Deadline: September 19, 2014)
a. Submit a completed Edison admissions application
b. Complete a COF essay titled, “Why I am interested in a STEMM discipline.”
c. Complete any required COMPASS testing or submit ACT scores.
7. This scholarship pays for tuition and fees (other than lab fees) only
8. Requires Edison enrollment of at least 6 credit hours
9. For renewal, planned participation in a “Learning‐by‐Doing” experience as certified by the appropriate

I authorize the release of all scholarship materials, including essays, transcripts, and testing scores to the members of
the Edison Scholarship Committee. If I am awarded a scholarship, I agree to abide by all the conditions of that
scholarship. I understand that if I fail to attend or officially withdraw from my classes my scholarship disbursement will
be removed from my account and I may be required to pay any resulting balance due on my Edison account. I
understand that this scholarship is for the Fall and Spring Semesters and that I must request renewal for future award
years if I continue to meet all eligibility criteria. I understand that this scholarship is not disbursed for summer and that
some scholarships will not disburse if tuition and other costs are all covered by Financial Aid or other funding.

If all eligibility criteria are being successfully met, submit a written essay (maximum of 500 words) titled “How I
plan to fulfill the learning‐by‐doing requirements in my STEMM discipline” to the appropriate Academic Dean or
Faculty Member for review and signature. Once signed, submit the essay and this application to Student
Services for further review.
