REQUIREMENTS FOR LICENSING OF A CORPORATE INSURANCE AGENCY For new applications of corporate agents: a) Complete Application Form (IAF 3) which requires specific information to be supplied, b) Certificate of Incorporation (Form 4), c) Memorandum & Articles of Association and/or Constitution of Company, d) Notice of Issue of Shares (Form 8), e) Shareholder certificates, f) Consent and Certificate of Director(s) or secretaries (Form 2B), g) Notice/Proof of physical and postal address (e.g. lease agreement, utility bills, etc), h) Agency agreement with the Principal Office, i) Evidence of insurance qualifications and experience (CV) of the principal officer; minimum CoP (certified copy ), or its equivalent, j) Certified copies of identification document (Omang for citizens; or work and resident permits, and passport for non-citizens); and 2 passport-sized photographs for the Principal Officer, k) An application fee of P1 200, l) Biographical Affidavit forms, Security Vetting Authorisation (SV1) forms and Police clearance certificates in respect of all controllers, m) Identification of controllers form. For corporate agent renewals: a) Application letter for renewal of licence along with the following information: i) List of active (existing) sub agents/consultants in the agency; ii) Confirmation of existing physical and postal addresses of agency (or any changes, if any), b) Copies of previous (expiring) licence, c) Confirmation of validity of agency agreement from the principal insurer, d) Renewal fee of P750.
