We would be grateful if you would read and sign the disclaimer / consent form below. This
form must accompany you / your child and be handed / filled in on arrival at the site.

I understand that A-Ball is an active sporting game involving the elimination of opponents via
the firing of A-Balls ( 0.68 calibre rubber / foam balls ) from a paintball gun. A-Ball games
take place on a purpose built arena with TigerTurf surface. I recognise that there are other
hazards on site; fallen trees, dead branches, sharp objects, holes etc and that A-Balls fired from
guns may bruise or break skin. I and or my child will conform to all safety rules in force on the
site, in particular the wearing of safety equipment which is issued on site and to obey all
marshals safety rules and instructions .
I hereby agree to my / give my consent for participation and absolve WDP Ltd & WDP
PAINTBALL Ltd, The National Paintball Fields their servants and agents in respect of personal
injury or death save where caused by the negligence of WDP Ltd and WDP PAINTBALL Ltd,
The National Paintball Fields their servants and agents and in respect of any damage to property
or goods howsoever this may arise or be caused. I confirm that I am 18 years of age or over
and am the participant or the legal parent / guardian of the person named above.
PLEASE NOTE : Persistent violation of our safety regulations eg: lifting of safety
goggles will result in the exclusion of the offending player/s from the games.
Our company policy states that all players must be aged 10 years or older in order to
take part in A-Ball games. By signing this form you as parent/guardian confirm that
your child will be aged 10 years or over on the date of play.
