Carolina One Player Intent Form
At Carolina One, we understand that tryouts are a stressful time for everyone involved. Our goal is to
place each player on the team that will allow for the best combination of both individual and team
success.
In order for Carolina One to commit to the proper players and coaches and to form teams appropriately,
we ask that you please indicate which C1 team levels you would be willing to play. You may check more
than one option if you are open to participating on the corresponding team level. Please consider
all of your options carefully and refer to our website for information regarding each level of play. This
form is non-binding and does not guarantee any player a spot on any C1 team.
You must turn this form in at check-in prior to the start of tryouts.
