Through a network of primary health care providers, it promotes and delivers optimal continuing health care to a population of over 600,000 residents in the eastern suburbs of Melbourne. Melbourne East GP Network is proud to be one of the first Victorian organisations to successfully become a Medicare Local. Since 1993 it has had a vision for enhanced community and primary care innovation, being a dynamic network delivering leadership in primary health care. Now trading as Inner East Melbourne Medicare Local it will continue to deliver the existing services and supports to GP’s and expand the provider support role across a much wider spectrum of professional service providers, encompassing the full gamut of providers contributing to the provision of primary care to patients. Servicing the municipalities of Boroondara, Manningham, Monash and Whitehorse, IEMML is responsible for driving improvements in general practice and primary health care to ensure that services are better tailored to meet the needs of local communities. As part of its vision, IEMML has developed a strategic plan to deliver enhanced primary health care across a number of areas including after hours care, aged care services, mental health and eHealth. To assist in the delivery of this plan, IEMML seeks an energetic and innovative person committed to improving community health outcomes and an interest in supporting local GPs and other primary healthcare service providers. Position Summary The position has as its key responsibilities: Coordinate a range of stakeholders including IEMML members, clinicians, PCP and other health organisations to collaborate in population health planning. Utilise the AML Alliance Comprehensive Needs Assessment (CNA) Framework to complete a CNA on behalf of IEMML. Develop strategies and resources together with a range of IEMML teams in the planning and development of evidence-based prevention andquality improvement activities. To provide leadership and training to relevant practicesand IEMML staff undertaking preventative health activities within general practice and other health organisations. To develop and disseminate high quality resources and activities that support a range of service providers in the area of prevention. d:\81924490.doc Page | 2 Work with the Manager, Project Management Office and other staff to ensure IEMML projects are driven by evidenced needs and priorities. To act as an ambassador for Inner East Melbourne Medicare Local. Key Selection Criteria Qualifications/Technical Expertise: Minimum 5 years working experience in a public health/health promotion discipline with relevant post graduate qualifications. Working experience in the general practice setting; epidemiology/biostatistics knowledge is desirable. Demonstrated ability to work with GPs and a diverse range of other primary care providers as needed. Experience in analysis of qualitative and quantitative data to determine health needs priorities including triangulation or cross check of results between consultations and data review. Experience in conducting needs analyses, reviewing population health data and/or compiling literature reviews. Demonstrated computer skills including MS Office programs and desktop publishing packages. Experience in project development, stakeholder engagement/consultation and management. Experience in workshop and forum facilitation. Business writing skills for the required audience. Desirable Knowledge and Skills: Experience working with GPs and health professionals, understanding of their professional concerns. Knowledge and interest in preventative health, chronic disease. Training and/or experience in epidemiology and biostatistics. Computer skills, especially related to general practice or medical software. Relevant social marketing and stakeholder engagement skills. Personal Qualities: Highly developed communication and interpersonal skills to work proactively and effectively with managers, staff and stakeholders. Duties, Responsibilities and Tasks Key Responsibilities: Population health planning Coordinate the liaison with representatives in local governments, community health services, primary care partnerships and other stakeholders. Continuing management of joint population health planning with these organisations. Responsible for completion of the IEMML Comprehensive Needs Assessment. Assist in identifying and keeping current the IEMML priority areas. Health Promotion Integrate health promotion activities into existing IEMML programs. Educate/promote the importance of health promotion to external coordinators of IEMML programs, and provide guidance on how to appropriately introduce health promotion principles. Assist upcoming IEMML projects to consistently apply a health promotion aspect. General practice engagement Undertake practice engagement to ensure regular identification of practice needs, communication about IEMML programs and initiatives and to act as a resource for the practices. d:\81924490.doc Page | 4 Develop and implement innovative prevention activities, integrated with other program areas, which will support general practice in adding value to and enhancing their practice. Work together with Practice Development and General Practice Liaison Teams in the planning and development of evidence-based prevention and quality improvement activities. Other duties as required. Salary is dependent on qualifications and experience. Generous salary packaging is available. Superannuation contribution is currently 9.25% of gross salary. Reimbursement for travel costs is paid for mileage incurred whilst on business (based on rates as prescribed by the Australian Taxation Office).
