Qualifying Major and Classification of Instructional Programs (CIP) code:
This 6-digit code is listed in the ‘PROGRAM OF STUDY’ section of your I-20. Look underneath ‘MAJOR 1,’ your major
will be listed there along with its CIP Code, which looks something like this: XX.XXXX (for Computer Science, use
11.0701; CIS & IT use 11.0103).
Level/Type of Qualifying Degree:
Masters or Bachelors
Date Awarded: (mm-dd-yyyy)
The date listed on your diploma.
If you completed your degree requirements in May 2016, that date is 05/07/2016. If you completed your degree
requirements in August 2016, that date is 08/05/2016.
Based on Prior Degree:
Enter “No” – if your application is based on your most recent degree and you are currently on OPT for that degree.
Enter “Yes” – if your application is based on a previous STEM degree, not related to your current OPT
authorization.
Employment Authorization Number:
This number can be found on your OPT/EAD Card. It’s the USCIS# that looks like this: XXX-XXX-XXX
(Be sure that you do not use the receipt number for your OPT application – receipt numbers usually begin with LIN
or YSC and should not be entered in this section of the application)
Form I-983 answers for Section 3: Employer Information – must be completed by employer
Start date of Employment: (mm-dd-yyyy) – If submitting new OPT STEM extension application
This is the day after your current OPT authorization ends, the date that your OPT STEM extension would begin.
Start date of Employment: (mm-dd-yyyy) – If already approved for OPT STEM and requesting to change
employers
This is the date that you will begin working for the new employer

Site Address (Street, City, State, ZIP):
Be sure to enter the address where you are physically working. This field is required by USCIS, and it will be the
address printed on your I-20 with your employment details.
Student Role, Goals and Objectives, Employer Oversight, Measures and Assessments:
Employer must thoroughly address all items requested in these sections of the form I-983. If information is
missing, edits will be required prior to the processing of a STEM extension I-20.
Student Role:
Specifically state what the student’s learning goals are. Specifically reference how job relates to certain classes
taken and/or how related to the STEM degree received at UCM.
Goals and Objectives:
A timeline of when goals are expected to be met must be included. This section should describe what is being
learned by the student, how, and by when.

Employer Oversight:
This section must provide details on how the student is being supervised.
Measures and Assessments:
This section must be specific to the plan and explain how student receives feedback on goals that student is
working to meet.
Evaluation on Student Progress:
This section must be left blank and should not be signed at the time of the initial OPT STEM extension application.
Students are required to submit one of the two evaluations after 12 months, and the other after 24 months of OPT
STEM extension approval.
