Student Role, Goals and Objectives, Employer Oversight, Measures and Assessments: Employer must thoroughly address all items requested in these sections of the form I-983. Measures and Assessments: This section must be specific to the plan and explain how student receives feedback on goals that student is working to meet. Evaluation on Student Progress: This section must be left blank and should not be signed at the time of the initial OPT STEM extension application. Students are required to submit one of the two evaluations after 12 months, and the other after 24 months of OPT STEM extension approval.
