Get other team’s Team Sheet with their best players and Goal Kickers Grab Club Votes from those doing them. Match Day Envelope with Umpire votes in envelope Umpires Report Match Day Checklist – original (Keep duplicate) 4 score cards 2 Team Sheets -1 from each team original given to Umpire Interchange Record You’ll only need a Report Form if someone is reported.
