SERVICE AVAILABILITY
This service is a 12 month pilot that is being
delivered in collaboration with Hereford City
GPs. The pilot will be evaluated regularly
throughout the 12 months to measure the
benefits.
Patients who are admitted to
Hospital at Home will be asked about their
experience of the service and any benefits
they believe they received.

What is Hospital at Home?
What happens next?
How will I benefit?
Hospital at Home is a service which will
care for your health needs in your own
home. The care that you receive will be
the same as you would receive in hospital.
The Community Matron will make
arrangement to get you home. This may be
with your family / carer or with the ambulance
service.
Patients are admitted to this service by
hospital consultants. The consultants will
see patients in A&E or on the wards and
they will talk to the patient and their carers
about Hospital at Home before they make
any decisions to refer to the service.
A member of the Hospital at Home team will
help you to settle back into your home
environment and make sure that you are
comfortable. The team member will talk to
you and your family / carer about the care plan
that was agreed by the Matron and Consultant.
You will know who will be visiting you, you will
know why they are visiting and you will know
when to expect them.
There is a lot of evidence that shows that you
will get better faster in your own home. This
service is trying to get you home sooner or
save you from being admitted in the first
place.
What happens if I am referred to
Hospital at Home by my Consultant?
The first thing that will happen is that you
will meet a Community Matron who will
talk to you and your Consultant about
what care you will need at home. The
Matron will assess your needs and put a
plan together which may include home
visits by nurses, therapists and care
assistants. It is really important that you
understand this plan and are happy with it.
Dependent upon how well you are, you may
receive several visits a day from different
health professionals from the team As you
become stronger and more independent the
visits will be reduced accordingly until you no
longer need the service. We will then
discharge you back into the care of your GP.
How long will I be a patient of
Hospital at Home?
The time spent under the care of Hospital
at Home will differ from patient to patient
and will depend on your progress. Some
patients will be with the service from as
little as a day or two, others may be longer.
A dedicated team of specialist staff will
provide health care, support and advice in the
comfort of your own home.
The care you receive will be tailored to your
individual needs.
Rapid response for clinical assessments and
patient visits, 24 hours a day, 7 days a week.
Information and advice about your condition
and its treatment will be made readily
available.
Reducing the likelihood of you requiring
hospital care in the future.
Friends and family are not restricted by
visiting times.
How do I contact the Service?
Will my GP know what’s happening to
me while I’m with Hospital at Home?
Yes, your consultant will let your GP know
about the care arrangements that we have put
in place for you.

This service is available 24 hours a day, 7 days
a week.
