STEP-BY-STEP – THE PROGRAM PROCESS
1st Step:
Sign up for a free information session by contacting our office at (503)227-5050 or via the website at
www.oregonweightlosssurgery.com. You may choose either a web seminar or a live, in-person seminar hosted by our
surgeon.
2nd Step:
Complete a new patient application (available upon completion of, or attendance at, an information
session) and submit to Oregon Weight Loss Surgery with a copy, front and back, of your current insurance card. Wait
for approval to being your pre-surgical evaluation appointments.
New patient applications may be submitted online, dropped off at the office, or mailed to the office at:

3rd Step:
We will verify your insurance benefits once your new patient application has been received.
If there IS a benefit –
We will contact you with
your insurance company’s
requirement list for
bariatric surgery coverage.
If there is NO benefit –
You will be mailed an
explanation letter and
information regarding
other financing options.
Self-pay patients –
You will meet with the Self
Pay Coordinator before
being referred for presurgical evaluations.
4th Step:
After you are approved, please call Oregon Weight Loss Surgery and speak to the New Patient
Specialist to schedule your pre-surgical evaluations and appointments. A full list of required appointments is available
on the Pre-Surgical Check List sheet (see next page).
5th Step:
Following the required pre-surgical evaluations we will schedule you for your consultation with the
surgeon. The surgeon’s scheduler will call YOU to schedule this appointment. Following the surgeon’s consultation
we will contact your insurance company for the final approval for surgery (called the surgery authorization).
6th Step:
Upon receipt of your surgery authorization we will notify you and schedule your surgery. You must
attend a pre-operative class within 2 weeks of your scheduled surgery date. At this class you will be given details
regarding your pre-surgery diet, discharge instructions to follow once you leave the hospital, and you will also be given
prescriptions for any medications that you will need upon release from the hospital.
7th Step:
Weight loss surgery!
8th Step:
Post-surgery follow-up is an important part of reaching your weight loss goals. Follow-up schedules are
dependant on the surgery performed and are available in this binder or at our main office. Studies have repeatedly
shown that long-term follow-up is essential in maintaining permanent weight loss.
