The Colorado Department of Health Care Policy & Financing (HCPF) is excited to announce the state’s
second year of participation in the National Core Indicators (NCI) survey. This survey is used by states
to measure system performance, service quality, individual client outcomes and client satisfaction and
compares that information with other states. The Department will use this information to improve the
quality of services in Colorado and across the United States.
________________________, you were one of 400 clients randomly chosen to participate in the faceto-face survey. Your participation in this important survey will help us to gather the critical information
needed to improve the quality of services provided to all individuals with an
intellectual/developmental disability. The interviewer will keep your information private and you will
not experience a change in services as a result of your participation. This survey will help the
Department to gain a better understanding of what is working well, what isn’t working well, and to use
that information to improve the quality of services.
