On behalf of Bill Manson, Acting Toronto Central LHIN CEO Hello, A Happy New Year to you and I hope you had a wonderful holiday season with family and friends. As we begin this new calendar year, I wanted to reach out to you to inform you of a new initiative across our LHIN looking at how we can better support integration opportunities for the benefit of patients and clients. As outlined in our 2015-2018 Strategic Plan, the Toronto Central LHIN is focused on delivering seamless, integrated care for patients and clients across the health system. The release of Patients First: A Proposal to Strengthen Patient-Centred Health Care in Ontario Discussion Paper reinforces the need for shared accountability as we work together to reduce system gaps and to strengthen patient-centred care. Aligned with this vision and our Strategic Plan, the TC LHIN is supporting a sector-led Integration Review initiative to identify frameworks and opportunities for more effective integration of services, built upon equity of access and local priorities, to drive better overall outcomes for our patients, clients and families here in Toronto. The Integration Review Initiative aims to build on the momentum of previous integration successes within our LHIN and beyond to identify better ways to support such activities to achieve the following: An evidenced-informed framework to identify opportunities for collaboration that will improve care and experiences for patients and clients in the TC LHIN; and, Support for services, programs and organizations who undergo voluntary and facilitated integrations across an integrations continuum, where the direct link between the results of individual integrations and TC LHIN system outcomes, population health outcomes, and value for money is clear. We know that this important work needs to be driven by and for the service providers across our community, and it has to be supported by evidence and experiences from your organizations. As such, I have asked two of your system colleagues, Anne Babcock, CEO of WoodGreen Community Services, and Marnie Weber, Executive Director, Strategic Developments of University Health Network to Co-Lead this important work, with the support of a Steering Committee reflecting a committed and experienced cross-section of leaders from our many partner agencies. Together, these individuals will be reviewing the work done to date, learning from others and engaging other system leaders to deliver back to the TC LHIN valuable insights in how to move our system forward successfully on integrations for the benefit of our patients and clients.
