UNMC follows the guidelines set by the CDC as well as the Nebraska Medicine’s Healthcare Epidemiology
Department for healthcare workers. Below is a list of UNMC required immunizations. Please make sure you have
all of these completed. If you are in the middle of a series, you will be asked to complete these during your first
school year. Please use this checklist to assist you and your physician to ensure you have met UNMC’s
immunization requirement. Please print off any record necessary that contains the dates these were administered
and results if applicable to use when submitting your immunization record online in MyRecords.
1. Two Measles/Mumps/Rubella (MMR) vaccinations and/or titers showing immunity to all three
a. If a titer shows you are not immune and you have documentation of two prior MMR vaccinations,
you will be asked to submit documentation of one MMR booster
b. If a titer shows you are not immune and you do not have documentation of two prior MMR
vaccinations, you will be asked to submit documentation of two MMR vaccinations administered 30
days apart
2. A current Tdap (tetanus/diphtheria/acellular pertussis) within the last 10 years. This must be a Tdap
vaccination and not a Dtap or other variation of this vaccination
3. Date from your most recent Polio vaccination if you have traveled outside of the Western Hemisphere
within the past 5 years
4. Two Varicella vaccinations or a Varicella titer showing immunity—a history of having the Chicken Pox does
not meet the UNMC requirement
a. If a titer shows you are not immune and you have documentation of two prior Varicella
vaccinations, you will need to submit documentation of one Varicella booster
b. If a titer shows you are not immune and you do not have documentation of two prior Varicella
vaccinations, you will be asked to submit documentation of two Varicella vaccinations administered
30 days apart
5. Tuberculosis screenings can be completed in a few ways:
a. Two current PPD skintests from consecutive years (current year, completed no more than six
months prior to registration, and most recent previous year, completed no more than one year
prior to registration) –OR—
b. Two current PPD skintests completed at least 7 days apart during the current year, completed no
more than six months prior to registration (a two-step PPD)
i. IF YOU HAVE EVER TESTED POSITIVE ON A PPD SKINTEST, YOU WILL NEED TO
COMPLETE (c) AND SUBMIT AN IGRA BLOOD TEST RESULT—A CHEST X-RAY DOES NOT
MEET OUR REQUIREMENT AND WILL NOT BE ACCEPTED WITHOUT A BLOOD TEST
c. One IGRA blood test from this year—this screening can be done even if you have not had a positive
PPD skintest in the past
d. TB testing is required annually and can either be completed as a skintest or IGRA blood test
depending on your TB status and preference
6. Documentation of three Hepatitis B vaccinations followed by a Hepatitis B surface antibody titer—OR—the
titer alone showing immunity
a. If a titer shows you are not immune, you will need to submit documentation of another entire
three-vaccination series of Hepatitis B given at the appropriate intervals. 30 days after your last
Hepatitis B vaccination of your second series, you will have another Hepatitis B surface antibody
titer drawn
b. If after documentation of two full Hepatitis B series you are still not immune, you will be asked to
submit documentation of a Hepatitis B surface antigen titer to see if you are currently infected with
Hepatitis B
7. Influenza vaccination is currently only required for College of Nursing students and needs to be completed
before December 1st each year, but is recommended for all students. If you have not had one done yet for
the current year, clinics are scheduled for the UNMC-Omaha campus in the Fall for students to have these
completed
