Terms and Conditions:
1. This LTW Challenge is open to all Nu Skin Singapore’s distributors.
2. The participant needs to lose at least 2.5 kilograms in body fat within 60 days from date of purchase as indicated on the invoice.
3. The participant needs to submit original invoice to Nu Skin Singapore as proof of purchase of the ageLOC® TR90™ System. The purchase of ageLOC® TR90™ System must be
made from 1 December 2014 to 28 February 2015.
4. The number of participants is limited to the names registered in the distributor’s account.
5. The participant who completed the 90 days ageLOC® TR90™ program is to submit his/her transformation results to [email protected]
6. In order to participate in this Challenge, the participant needs to submit a completed participation form and letter of consent and authorization to Nu Skin Singapore Showcase,
Product Collection Counter. Incomplete submission will be disqualified.
7. The “Before” and “After” body fat analysis records will be taken according to the reading of Omron Fat Analysis device.
8. The information submitted must be truthful and accurate.
9. Free items are neither refundable nor exchangeable. No PSV and CVS entitlement for free items.
10. Nu Skin Singapore reserves the right to conduct a body fat measurement on the participant and disqualify any participant who does not fulfilled the prize winning condition based
on Nu Skin Singapore measurement.
11. Nu Skin Singapore reserves the right to amend the terms and conditions of this Challenge and substitute the prize(s) with item(s) of comparable value(s) in its sole discretion, without
prior notice.
12. Nu Skin Singapore reserves the right to make the final decision in the event of any dispute on this Challenge.

LETTER OF CONSENT AND AUTHORIZATION
To: Nu Skin Singapore Enterprises Pte Ltd. (the “Company”)
I, _____________________________(Name as in NRIC), SG ID: ___________________, agree to submit my 90 days
transformation result of ageLOC TR90 LTW Challenge to the Company within 14 days from the completion date of the ageLOC®
TR90™ program. I declare that the transformation result and transformation story I provided (which include but not limited to my
name, photos, professional title(s) and affiliation(s), likeness and statements) (the “Testimonial”) describe accurately my experience
with using ageLOC® TR90™ System (the “Products”).
I hereby consent and authorize the Company, together with its parent company, its affiliated companies and related entities
(hereinafter collectively the “Nu Skin Group”) to have the right to use at all times my name, photos, professional title(s) and
affiliation(s), likeness and statements (including but not limited to this Testimonial) in connection with my sharing of experience in
using the Products for any and all purposes set out in the Company’s Policies and Procedures and the Company’s Personal Data
Protection Notice (which include but not limited to the purpose of marketing and promoting the Products), and in all forms, whether
or not in the form of publication in videos, VCDs, DVDs, Nu Skin Group’s magazines, websites and any other audio-visual or printing
materials. Also, I hereby authorize that all rights and benefits acquired by Nu Skin Group hereunder shall apply throughout the world.
For the avoidance of doubt, I hereby waive my rights against Nu Skin Group of and from any and all fees, expenses and compensation,
and any and all other causes of action whatsoever arising out of or resulting from whatever use of my name, photos, professional title
and affiliation(s), likeness and statements (including but not limited to this Testimonial). I shall hold Nu Skin Group and all of their
agents, officers, directors and employees free and harmless from any liability arising from claims or suits of third parties, including costs
and expenses incidental thereto, in connection with the use of the Testimonials.
I understand that the authorization given pursuant to this Letter of Consent and Authorization (this “Letter”) may be revoked in writing
at any time, except to the extent that action has been taken in reliance on this authorization. I also understand that this Letter
supersedes all other agreements and/or understandings, both verbal and written, regarding the Testimonial and that there is no
guarantee that the Testimonial will be used in any production, publication or broadcast.
I hereby agree that all the terms and conditions of this Letter shall not only cover this Testimonial, but also all the future testimonials
(as indicated by my signature on each and every page of such future testimonials) provided by me to the Company and/or Nu Skin
Group from time to time.
