Please read the information below, then sign and return the bottom portion by
Tuesday April 4, 2017.
Field Trip Information
Day: Thursday
Date: April 6, 2017
Location: Lubbock Science Spectrum
Purpose: To visit the Science Spectrum museum, Omni theater and Dinosaur Exhibit
Pre-K 3 - 5th graders
9:30am - 1:30pm
Cost: $10
Means of transportation: Slaton ISD has graciously granted us a bus, parents are
welcome to join us, but please reserve your seat on the bus by signing below. Any
parents wishing to drive their own child please indicate below.
Time departing SJS: 9:00am
Time arriving at SJS: 2:00pm (approx.)
Special Instructions: Students should wear jeans and 2016/2017 school t-shirts
Pack a sack lunch, we will be eating in the lunchroom at 11:30.
Save top portion of form for reference.
