Inclusion criteria
• Without this intervention hospital
admission would be unavoidable
• The Patient has agreed and consented
for treatment to be provided by Acute
Care at Home
• The Patient must be 18 years or older
• The Patient has telephone access at home
for support and / or in case of an emergency
• A GP needs to have assessed community
referred patients within 24 hours of
referral and will take medical responsibility
for the Patient
• If out of hours, a Cornwall Out of Hours
clinician will maintain responsibility until the
Patient’s own GP surgery resumes in-hours
responsibility
The Acute Care at Home Team is
available 8am-10pm, 7 days a week.

Where we are
The Acute Care at Home Service is provided
by a team of nurses and offers high levels of
nursing support across Cornwall.
• Complex patient assessments for
admission prevention and review
pre-discharge from hospital
• Administration of drug therapies including
IV antibiotics, anti-emetics, diuretics and
fluids for rehydration
• Cannulation and phlebotomy
• Management of central lines and
implanted ports
• Respiratory assessment and examination
• We can offer short term nebuliser loans
and monitoring of oxygen therapy
• Cardiovascular assessment (ECG machine
available if required)

Our aim
To deliver advanced nursing care and support
to patients in their own homes; to prevent
hospital admission and facilitate early discharge.
What we do
We accept referrals from Healthcare
Professionals in Acute and Community settings
and can discuss patients on an individual basis.
Responsibility for the Patient remains with
the GP / Consultant who referred the Patient.
Packages of care can be arranged via Adult
Care and Support or the Early Intervention
Service (EIS) if appropriate.
• Following assessment / treatment the team
will inform the referrer of any changes and
liaise with them to discuss further treatment
plans and / or discharge from the service.
• The Acute Care at Home Team is available
from 8am-10pm, 7 days a week.
This allows us to treat
conditions including:
• Exacerbations of COPD
• Chest infections / Pneumonia
•Cellulitis
• Urinary tract infections
• Some cardiac conditions such as Heart Failure, Atrial Fibrillation
• Post-operative treatments / care
