It’s important to get started right away to avoid delaying your start date. Location Affects How You’ll Complete, Return Forms How close you live to a Chase New Hire Welcome Center, or LabCorp testing facility determined how we asked you to complete and return your Pre-Employment Forms. You may be providing information online, or completing paper forms which you’ll bring to a scheduled appointment or return by overnight courier (FedEx or UPS). Refer to your email from JPMC Staffing with “Welcome to Chase! Complete Your Pre-Employment Process Soon” as the subject line to confirm if you have a scheduled pre-employment appointment. Using This Guide The first section of this guide gives you step-by-step instructions on completing the following 5 pre-employment requirements: 1 Complete Online Onboarding Forms 2 Sign Offer Letter accepting employment 3 Be fingerprinted 4 Take a drug test 5 Complete an I-9 Form The second section of the guide, Your Pre-Employment Forms, starts on page 7 and includes the forms you may need to complete. For easy reference, the step numbers in the instructions match up with the numbers in the forms section. If you downloaded a PDF copy of this guide, you may need access to a printer. The final step, Return Your Pre-Employment Forms, provides a quick reference for how you’ll return the information back to Chase that you’ve worked so hard to complete in 24 hours. Have questions? Contact our Pre-Employment Team for help toll free at (877) 332-4473 Team members are available Monday through Friday, 8 a.m. to 8 p.m. Eastern Time to answer your questions. 2 Welcome to Chase! 1 Complete Online Onboarding Forms Pre-Employment Guide At the time you were offered a job at Chase, you received an email from JPMC Staffing that included a link to the Chase Onboarding Site. By now you should have visited the site and completed your online onboarding forms. If you indicated that we should NOT contact your current employer: • Email a copy of your W-2 and a pay stub to: [email protected] • OR fax a copy of your W-2 and a pay stub to: (866) 378-1440 • You’ll receive a confirmation that we received you email or fax within three business days. If you’ve completed your online onboarding forms, skip to step 2, Accept Offer of Employment. If you haven’t completed this important step, do it now. Locate the pre-employment e-mail you received from JPMC Staffing for a link to the Onboarding Site and your temporary user name. Use the password you created when you applied for this position online. If you’ve forgotten your password, enter your user name and click on the ‘Forgot Password’ link to receive a new, temporary password by e-mail in about 30 minutes. You can also call 1-866-473-6765 to receive a temporary password or for technical assistance. You’ll need the following information available to complete your online forms: • Previous employment details - company name, address, phone number, and dates of employment • Education details - name of school, address, dates you attended • Previous home addresses • Emergency contact information • Account information if you choose to receive your pay by direct deposit. 2 Accept Offer of Employment You received an email from Chase with the subject Offer of Employment For Chase. The email contains a link to your Employment Offer. You’ll use your email address to log in to the online form. Follow the simple instructions to verify your identity, create an electronic signature and view your Employment Offer. Once you’ve reviewed and accepted the terms of your offer, you’ll submit your formal acceptance online. 3 Welcome to Chase! 3 Be Fingerprinted Pre-Employment Guide As a condition of your employment, you are required to be fingerprinted. Fingerprints are used to complete a criminal background check. The results may affect your eligibility to work at Chase or your start date. If you were scheduled for electronic (LiveScan) fingerprinting: • At a Chase or non-Chase facility: Bring a valid photo I.D. You don’t need to complete the LiveScan Electronic Fingerprint Information Form. If you were asked to complete fingerprint cards: • Two fingerprint cards were included in your package. L DON’T FORGET Regardless of how you’re being fingerprinted, you’ll need to have a valid, government-issued photo I.D. with you when you go to your appointment. • You can take your fingerprint cards to any location that will use the cards you received to record your fingerprints. Typically, a law enforcement agency or notary will provide this service. There may be up to a $25 fee. You should call ahead to schedule an appointment, check on fees and acceptable payment options. Be sure to ask for a receipt. On your first day of work, give the receipt to your manager so you can be reimbursed for the cost. • Before you go, it’s important to refer to the highlighted sections on the sample fingerprint card (see page 8) and complete the corresponding sections on your fingerprint cards. Please use black ink and print legibly. Illegible writing will delay processing. You’ll also need to take a valid photo I.D. • Return Both fingerprint cards to Chase in the overnight courier (FedEx or UPS) return envelope you received. Need more than 24 hours? If your local law enforcement agency only does fingerprinting on a specific day, please return your fingerprint cards as soon as possible. 4 Welcome to Chase! 4 Take a Drug Test Pre-Employment Guide As a condition of your employment, you are required to take a drug test. The results of your drug test may affect your eligibility to work at Chase or your start date. You’ll need to visit an approved lab for your drug test. You’ll need to take the Chain of Custody Form with you. Chase uses two different Chain of Custody forms based on your location. If you received a multiple page form (Overnight Package): • Locate the closest partner lab to you on the list included with the form. • Before you go, call the lab you have selected to confirm they perform occupational urine drug screen collections and the hours the lab is open. If you did NOT receive a multiple page form: • Print the Chain of Custody form on page 9 of this guide. • Locate the closest LabCorp lab by visiting http://www.labcorp.com/psc/index.html. Please make sure that you select “occupational urine drug screen collection” to ensure that the lab you plan on going to, conducts pre-employment testing. 5 Complete an I-9 Form As a new Chase employee you must establish your identity and valid employment eligibility by completing an electronic I-9 Form. If you’re scheduled for an appointment: • You’ll need to complete section 1 of the online I-9 form before going to your appointment. You’ll receive an email from our I-9 vendor, CARCO Group. This email will have a link to their website as well as your first time username and password. You will receive this email 24 hours after completing your Onboarding Forms. Please make sure you complete Section 1 of the I-9 before your onsite appointment. Please bring original forms of government issued identification for your appointment. The online site has a full list of acceptable IDs. If you have NOT been scheduled for an appointment: • You’ll need to complete section 1 of the online I-9 form before your first day. You’ll receive an email from our I-9 vendor, CARCO Group. This email will have a link to their website as well as your first time username and password. You will receive this email 24 hours after completing your Onboarding Forms. Please bring original forms of government issued identification on your first day as you will be completing this with your hiring manager. The online site has a full list of acceptable IDs. 5 Welcome to Chase! b Return Your Pre-Employment Forms Pre-Employment Guide Where you live determined if you have a scheduled pre-employment appointment or if you received an overnight courier (FedEx or UPS) package, as well as how you’ll need to return your pre-employment forms. Here’s a quick reference for the final step of the process: If You Have a Scheduled Appointment and DID NOT Receive an Overnight Package You’ll need to bring the following with you to your appointment: • Acceptable forms of identification for I-9 If You Received an Overnight Package L You’ll need to return some of the documents to Chase in the overnight courier (FedEx or UPS) return envelope you received as part of this package. The rest you’ll bring with you to your appointment. DON’T FORGET If you indicated that we should not contact your current employer, AND: Return in the Overnight Courier (FedEx or UPS) Envelope: • If you received fingerprint cards, return both completed cards. Are scheduled to meet with the Pre-Employment Team, bring a copy of your W-2 and a pay stub to your appointment. Are NOT scheduled to meet with the Pre-Employment Team, email a copy of your W-2 and a pay stub to [email protected] OR fax a copy of your W-2 and a pay stub to: 866-378-1440. 6 Your Pre-Employment Forms Please complete within 24 hours Using the instructions provided in the guide, please complete the following forms: Completed online 4 Received by e-mail 4 1 Onboarding Forms 2 Offer of Employment Letter 3 Fingerprint cards 4 Drug Test Collection Authorization & Chain of Custody Form 7 Pre-Employment Forms 3 Fingerprint Cards If you received two finger print cards with this package, you’ll need to fill in the sections of the card outlined with a solid line on the picture of the card below.
