The Supported Living Team is funded by Supporting People and was set up within Bron Afon to support members of the community who are 50+ and have a Housing related support need. We are looking to recruit an enthusiastic and motivated individual to our team, in the role of Support Officer. You will be responsible for delivering a comprehensive needs led, Support Service which enables older people to, wherever possible, remain within their home and lead an independent, full and varied life. You will need to be experienced in providing a person centred/ needs led support service in the community, carrying out needs assessments, developing support plans and risk assessments, and enabling service users to achieve their goals. Bron Afon will be required to carry out an enhanced DBS for the candidate that is appointed to this role. Implementing the Bron Afon Community Mutual Model 2.1 Play a key role in ensuring that Bron Afon’s Support Service implements the Bron Afon community mutual model, through effective community involvement and positive partnership working across all sectors and within local communities. 2.2 Work effectively across Bron Afon, to ensure that excellent relationships are developed and maintained between all individuals, teams, service areas and other partner organisations. 2.3 Personally engage in learning and development projects that are relevant to the post and advancing the services which Bron Afon provides. 3. Support Service 3.1 Deliver services which ensure that all levels of support needs, including Complex Needs, of older people are assessed and met in a way that maximises independence and adds to individual’s quality of life and are in line with Supporting People Framework, Policies and Procedures. 3.2 Actively provide advice, assistance and where appropriate act as an advocate to ensure that older people access timely support and the right services from the relevant health, social, housing and voluntary sector agencies. 3.3 Ensure that all relevant service user data is assessed and managed, and that up to date and accurate records are maintained. 4. Managing Resources and Performance 4.1 Provide an excellent support service ensuring that the relevant outcome measures, records and data are in place to demonstrate how this service is meeting or exceeding agreed target and key objectives. 4.2 Proactively manage and mitigate risks, ensuring that all health and safety requirements are fulfilled and any regulatory inspections are carried out. 5.
