SCENARIO 1
1.
IOCL LPG bottling plant is storing liquid LPG in three horizontal bullets and two
Horton spheres under atmosphere temperature. At about 4.15 hrs on 20 December
2008, a leak of LPG was noticed due to failure of one of the flanges (an
engineering fault) of Horton Sphere by the operational staff on duty.
2. Due to leaking of the LPG outside the boundary of IOC LPG plant, the gas came out
of the plant boundary and caught fire due to spark from one of the truck passing
outside the plant boundary and after a while this fire led to BLEVE in the one of the
Horton spheres:
(a) Inside Plant –3 killed and 12 injured.
(b) Outside Plant– 5 killed and 37 injured.

Stakeholders.
SITUATION 1
1. IOCL terminal stores, handles and processes a number
of hazardous
inflammable petroleum products at atmosphere temperature and pressure.
At about 5.45 hrs on 20 December 2008, there was a catastrophic failure of a
tank storing MS and got fire due to impact of BLEVE at neighbouring LPG
Bottling Plant, as cascading effect. Due to fire in the MS tank, the impact is
as follows:
(a) Inside IOCL terminal – 2 killed and 10 injured.
(b) Outside Terminal - 2 killed and 28 injured.
Live stock – killed 28 and affacted – 35.
Excessive damage to crop nearby.
2. Due to BLEVE at IOCL LPG plant the Tankers parked outside near to bulk
storage terminal also caught fire and the impact is follows:
(a) Three Tanker Trucks caught fire
(b) 5 killed and 22 injured due fire

SITUATION 3
BPCL terminal stores, handles and processes a number of
hazardous inflammable petroleum products at
atmosphere temperature and pressure.
At about 6.00 hrs on 20 December 2008, two persons
were challenged by the security staff in the piping and
pumps area in the BPCL terminal. They were wearing
the BPCL staff dress. One of them removed his coat
and suddenly there was a loud blast, killing the suicide
bomber to pieces. While people rushed towards the
incident site, the second person fired from his AK 47
rifle, killing 3 security personnel & injuring 8 staff
members of BPCL. The blast affected the pipes
rupturing a number of them causing leakage . The
firing by the second terrorist caused pool fire which
spread to the MS and Diesel tanks.
REQUIREMENTS
1. As security staff, operational staff, fire services and the
management of BPCL, actions to be taken.
2. Action to be taken by district administration after’off
site’ emergency is declared. Role of Fire & Emergency
Services, Police, Medical Services, NGOs, Civil
Defence, other Stakeholders.
3. Where will the ICP be located and who will be the
overall Commander.
4. Where will be the Relief Camp established?
