Please send us back the letter (included in the last page of this letter) signed off for acceptance of required
actions including:
 acknowledgement about this notification;
 CSL mandatory information (CSL contact person; CSL site/lay-out, CSL instruction and short description
of the inspection method, special identification of product and packaging).
Review of CSL status will be conducted weekly in order to confirm action effectiveness or needs for further
escalation according to the Quality First Manual (http://www.keysafetyinc.com/quality.asp).
In case of escalation to CSL 2, you will need to notify your Certification Body within 5 business days after being
placed in Controlled Shipping Lev. 2 Status as per KSS Procedure #1005485.
The actions required to exit from CSL1 are as follows:
 30 consecutive working days or 5 consecutive deliveries without problems (whatever is more severe);
 Updated problem solving report to confirm corrective action effectiveness.
