This team is specifically for athletes with special needs.

AGES: 5 TO 15
REQUIRED FORMS: Medical form, player’s contract, 2012 work bond form, code of
conduct, wallet size picture, and report card or 1st page of IEP. All forms can be
downloaded on our website @ www.southernramsayf.org
Registrations: April 22nd and May 20th Ocean Acres Community Center 9:00 am –
12:00 pm
