Please complete the form electronically.
Information for applicants
Please ensure that when you save (and later send) both your completed application form and supporting
evidence that the file names includes the words ‘Excellence Award’ and your organisation’s name.
E.g. ‘Excellence Award_Trainer_A.doc’
Evidence – You may attach evidence to support your application in a separate file of
approximately 20 x A4 pages
Ensure that this file is named to include the words ‘Excellence Award’ and ‘Evidence’ and your
organisation’s name. E.g. ‘Excellence Award _Evidence_Trainer_A.doc’
When completing Section 2, please ensure that you make reference to your supporting evidence for
each of the dimensions, and that you clearly signpost to that evidence.
Section 3 gives you an opportunity to number and list all your evidence.

Dimension 1: Delivering the Strategy
Give examples of how you meet the following criteria and how this supports/leads to better quality of
life outcomes for people who use adult social care services:
a. How do you critically review relevant thinking, ideas and best practice and apply whole systems
thinking in order to conceptualise a strategy in line with the vision of learning and development?
b. How do you integrate the views of a broad range of stakeholders to develop a coherent and
sustainable strategy, assessing its readiness for change and manage the risks, political
sensitivities with environmental uncertainties?
c. How are systems, processes, resources and plans put in place to deliver the strategy and
accountabilities and demonstrate flexibility when changes are required?
d. How do you support the conditions and culture needed to sustain changes integral to the
successful delivery of the strategy, and evaluate outcomes and using learning to adopt strategic
and operational plans?
Potential evidence:
Excerpts from strategic plans to show how they meet the vision of the organisation.
Examples of how a consultative culture is created to support delivery of the strategy and to drive
strategic change within the wider social care system.
Examples of how strategic outcomes are monitored, evaluated and adjustments made as
necessary to ensure the sustainability of the strategy.

Dimension 2: Embedding leadership qualities
Gives examples of how you meet the following criteria and how this contributes to improved leadership and
better quality of life outcomes for people who use adult social care services:
a. How do you embody a high standard of professional behaviour in every aspect of your organisation,
whilst creating and reinforcing professional standards?
b. How are your learning and teaching techniques used to encourage all learners to be self-aware
whilst showing compassion and empathy?
c. How do you demonstrate and champion leadership and encourage the fundamentals of coproduction and positive communication leading to continual improvement?
Potential evidence:
Evidence of integrating tools to develop self-awareness in all course design and delivery and
evaluation.
Evidence of a commitment to adult social care related personal and professional development.
Evidence that people who use services are an integral part of the design, delivery and evaluation
of all learning materials.
Evidence of collaboration and the sharing of best practice with other providers and involvement in
relevant adult social care networks.
Evidence of the design, delivery and evaluation of all learning materials.
Evidence of working closely with adult social care providers to develop courses and materials that
develop and improve leadership behaviours within the sector.
Evidence of the completion of the individual self-assessment questionnaire for all staff including
associates.
Evidence of a development and improvement cycle which includes feedback from learners,
employers and people who use services with appropriate actions taken on board and
implemented.
Evidence of Policy or procedure / flow chart that clearly outlines the organisation’s resource
management strategy.
