F2 Emergency Medicine
The Emergency Department at the
Luton & Dunstable Hospital provides a
comprehensive accident & emergency
service for the South Bedfordshire
District which has a population of
approximately 350,000.
In 2010/11 there were over 590,000 first
attendances and 1,000 follow-ups.
Approximately 18% of the new
attendances were children.
F2 doctors will be seeing a wide
spectrum of presentations across all age
groups. There is a need to act on limited
information, under time pressures and
unpredictable number of patients. There
are senior doctors in the department at
all times who will advise and help.
The tasks of a F2 doctor provide a neat
list of learning opportunities which are
continuously supervised.

Emergency department at L&D Hospital
F2 doctors are expected to work in all
areas of the department.
Resuscitation area: known as ‘Resus' –
is the nerve centre of the Emergency
Department and is used for the
immediate treatment of patients
suffering from trauma or those suffering
from life-threatening, medical or surgical
emergencies.
Trolley area: known as ‘Majors’ – is the
engine house of the Emergency
Department and is used for stable
patients who need assessment and
treatment on a trolley prior to
disposition.
Minor Injuries & Illness Area: Minor
injuries and illness are treated in the
Emergency Department within a new
dedicated area, which has been built
specifically to meet the needs of
patients needing this level of care.
Paediatric area: Building of this new
area has been completed. Children not
requiring resuscitation will be managed
in this area.
Note: There is an ongoing pilot of GP
led urgent care services for minor
illness.
Trainees will be allocated an appropriate
clinical supervisor
“…initial evaluation, treatment, and
disposition of any person at any time for
any symptom, event or disorder deemed
by the person – or someone acting on
his or her behalf – to require expeditious
medical, surgical or psychiatric
attention” Clinical Practice of emergency
medicine as described by Hamilton. To
deliver on this philosophy whilst
acknowledging the tremendous
challenges and opportunities it creates.
Typical working pattern in this post
F2 doctors work the junior doctors
Rota. This is a 7 day on & 2 day off
Rota. 7 days are worked
consecutively with 2 rest days after
each 7 days and 9 rest days after 7
nights.
The following shifts apply: 7am16.00, 15.00-00.00 and the night
shift 23.00-08.00am.
Didactic teaching in protected hours
happens twice a month for 2 hours.
This is consultant led teaching and is
of very high quality and directly
relevant to Emergency Medicine

The department
There are currently eight GP Practices
based in Luton, Harpenden or
Edlesborough that offer placements for
F2s. Trainees will be allocated a
practice once they have ranked the GP
practices.

Placement
F2 Medical Rotation
The department
All F2 trainees undertake a medical
rotation comprising of 9 weeks on either
a medical specialty ward or on a
Medicine for the Elderly ward and 9
weeks on the Emergency Assessment
Unit.
Medical Specialties – there are
approximately 114 beds allocated for
general medicine and related
specialties. F2s will be attached to one
of the following specialties; Respiratory,
Rheumatology, Cardiology, Diabetes &
Endocrinology, Gastroenterology
EAU – all medical patients on the EAU
are managed under the care of the three
Acute Physicians. Patients requiring
longer stay or care in a specialist ward
are transferred to the appropriate unit
within 48 hours of admission.
The Medicine for the Elderly team offers
in-patient and out patient services for
older patients with general medical and
complex geriatric problems. It holds
daily outpatient clinics. The directorate
has six wards (St. Mary’s) with a total of
148 beds and seven consultants.
EAU
As the FY2 in EAU you are responsible
for clerking into hospital medical
patients either from GP referrals or via
A&E. These are busy shifts but provide
a wide variety of cases and experience
in managing many of the common
medical emergencies. You work
alongside a Core Trainee and the on
call Medical Specialty Trainee. During
the 9 week EAU rota you will cover the
Medical Short Stay ward – ward round
duties and facilitating discharges.

Base Ward
This work depends on the sub-specialty
you are covering but the basic work for
all in-patients is the same. A daily ward
round is necessary to prioritize
treatment and organize discharges. It
provides the opportunity for multidisciplinary work and trainees can gain
experience in both acute and chronic
conditions. Consultant ward rounds are
an opportunity for on the job teaching
and there are numerous opportunities
for this. Opportunities exist to attend
out-patient clinics and involvement in
the teaching of medical students and
F1s is expected.

helping FY2 on ward

Placement
F2 Obstetrics & Gynaecology
The department
Gynaecology has been an established
specialty at the L&D since the hospital’s
foundation in 1939. The department
offers one of the most comprehensive
Gynaecological services in the region.
The Maternity team at L&D provides a
comprehensive service to women in
Luton, south Bedfordshire and
Hertfordshire and in 2010 successfully
delivered over 5,000 babies.
The type of work to expect and
learning opportunities
The F2 doctor is responsible with other
staff for the ward care of patients and
the maintenance of the patient’s medical
record. They will be involved in looking
after and reviewing postnatal and
gynaecology patients on the wards.
There will be opportunities to work with
consultants in out-patients clinics one
day a month.
On call an F2 doctor will be involved in
managing obstetric patients and will
clerk gynaecology referrals from A&E
and GPs.
There are lots of opportunities to be in
theatre and improve surgical skills.
The F2 doctor will be responsible for
such other specific clinical duties as
allocated by consultants including
performing other duties in occasional
emergencies.

Placement
F2 Ophthalmology
The department
General Ophthalmic Service within a
District General Hospital. The majority of
the patients are treated for cataract,
glaucoma and medical retinal conditions.
Sub-specialty clinics are provided in the
areas of glaucoma, corneal diseases,
vitreo-retinal conditions and paediatric
ophthalmology. Argon & YAG laser
treatments are carried out. There is a
dedicated eye theatre with lists 11
sessions per week and minor operations
are performed in clinic.

The two F2 doctors attached to the
learning opportunities
ophthalmology department are
responsible (with other staff) for the
mostly outpatient-based care of patients.
They will take part in timetabled outpatient
clinics and will develop ophthalmic history
and examination skills. On gaining
sufficient training, the F2s will be
responsible for several of the weekly
Acute Eye Clinics. Within theatre, there is
the opportunity to learn simple practical
procedures. During the 4 months each F2
is expected to complete a clinical audit.

Placement
F2 Paediatrics
The L&D’s paediatric services are part
of the Women’s and Children’s Division.
Our Paediatric Unit has 54 beds,
including a 5-bed Paediatric Admissions
Unit for children up to the age of 16
years.

The F2 doctor will be responsible for the
learning opportunities
care of patients on the general
paediatric wards with the experienced
multidisciplinary team. They will also
work as part of a team on the acute
paediatric assessment unit clerking,
assessing
and
managing
new
admissions. Consultant led outpatient
clinics are held regularly and there is
ample time allocated on the rota to
attend these. There is a good structured
teaching programme which attendance
will be expected. F2 doctors are able to
teach at the weekly grand round
meetings which provide a good
opportunity to discuss interesting cases.
Occasionally F2 doctors will be
expected to perform other clinical duties
allocated by consultants in unforeseen
circumstances or emergencies. The job
does not involve time on the Neonatal
Intensive Care Unit.

F2 General Surgery
The department
The type of work to expect and
learning opportunities
General Surgery
The F2 doctor is responsible with other
staff for the ward care of patients and
the maintenance of the patient’s medical
record. They will have opportunity to
work with the consultants in theatres as
much as they desire provided that ward
work is managed appropriately. They
will also have the opportunity to attend
outpatient clinics and see their own
patients to make decisions on booked
elective procedures. They are expected
to attend the structured teaching
programmes provided by the
department, and grand rounds. The
doctor will have regular on-calls where
they will clerk and manage patients until
review by seniors, and at night are
responsible for all surgical and ENT
admissions, as well as helping the FY1
when they ask for help on the wards
with difficult patients.

The surgical block at Luton & Dunstable
Hospital was opened in 1983. There are
three surgical wards with additional
outlying patients on the other wards,
HDU and ITU. The main theatre suite
contains six theatres which are well
equipped for laparoscopic surgery.

F2 Trauma & Orthopaedics
The department
The Trauma & Orthopaedic department
treats both elective and emergency
patients with over 5,000 Orthopaedic
surgical procedures carried out in 2010
for patients from Bedfordshire,
Hertfordshire and beyond.

The F2 doctor is responsible with other
learning opportunities
staff for the ward care of patients and
the maintenance of the patient’s medical
record. They will have opportunity to
work with the consultants in outpatients
clinics for at least one day each week,
and attend the consultant’s theatre list at
least two sessions per week. They are
expected to attend the structured
teaching programmes provided by the
department. The doctor will be
responsible for such other specific
clinical duties as allocated by
consultants including performing other
duties in occasional emergencies and
unforeseen circumstances.

L&D Hospital – with 30 adult beds on
ward 23. There is a dedicated day case
ward with 17 beds nominated for use by
the Orthopaedic Department which
connects to a day surgery theatre.
There is a further 20 bedded elective
inpatient ward (ward 2).One of the
Trust’s operating theatres is reserved
exclusively for Orthopaedic cases and is
equipped with lamina flow air supply for
joint replacements.
