Nottingham Hoods, Notts Nova & Nottingham Wildcats would like to invite you to provide a team to

take part in our 2 annual 3on3 Tournament at Nottingham Wildcats Arena on 28 June 2014 from
10am to 6pm.
Entries must be made from teams of 4 (3 players with a sub). Players cannot enter more than one
age category or play for more than one team. Team entry costs £16, working out at just £4.00 per
person. Teams will play a minimum of 3 games on the day.
Enter your team today by downloading the entry form and returning by email to Linda English,
Austin Grant or [email protected] or [email protected] and pay cheque,
cash, hand in the entry form and money to any of the above. If you prefer to pay by BACS then
contact one of the above for details of how to do this.
If you have any questions regarding the event or entry into the event please do not hesitate to contact
any of the above. Competition rules will be available on www.hoodsbasketball.com;
www.nottinghambasketball.co.uk or www.novabasketball.com and will also be displayed during the
tournament day.

1. Complete page three of this entry form, indicating your age category and team/player details.
2. Register your team by completing this form, then either hand it to Linda English or Austin
Grant, or email to: [email protected]; [email protected]; or post to
Linda English, 43, St.Mawes Avenue, Wilford, Nottingham, NG11 7BX. Registration costs £16
per team.
Payment must be made at the time of submitting your entry form. Two options to pay either by
handing your entry form and cash or cheque - made payable to Nottingham Hoods Basketball Club to
any of the above. If you prefer to pay by BACS then contact one of the above for details of how to do
this.

 Rules will be available on www.hoodsbasketball.com; www.nottinghambasketball.co.uk or
www.novabasketball.com and will also be displayed on the day of the tournament
 Team organisers will receive an information email with specific tournament details such as
rules, directions and schedules nearer to the event.
 Each team will play at least 3 games during the day.
 Players can only compete in one team and one age group during the tournament.
 Teams must have a light and dark shirt per player.

Photography and filming
On completion of a registration form, photographers will be permitted to take recorded images at the
event, in line with the guidance issues by England Basketball, which will be available online and at the
event. If you are concerned about any photography taking place at this event, please contact the
Event Welfare Officer.
