The player identified overleaf is appropriately registered with the
Ontario Soccer Association (OSA), and is a member of the identified
team playing in the Valley United Soccer League (VUSL). For the
season identified, the signatures of the team and club officials grant the
player permission to participate as a guest player with other ‘higherlevel’ teams from the same club in VUSL games, in accordance with
and subject to the policies of the OSA (Section 6, Policy 8.1) and
VUSL (Section IV).

Section 1 of this form is completed by an official of the team (i.e.,
coach or manager) to which the player is registered. Section 2 is
completed by an appropriate official of the player’s club (e.g.,
President). Both sections 1 and 2 must be completed and signed
before the registrant participates as a guest player in any VUSL game.

This form accompanies the player to each game in which he or she
participates as a guest player. An official of the team to which the
registrant is playing as a guest must complete section 3 (VUSL Guest
Player History), one line for each game played. VUSL rules limit a
registrant to at most five games as a guest player, per season.
In addition, the guest player’s name and OSA ID# must be added to
the team game sheet, and the game sheet must identify the player as a
guest.
