What is an Integrated Accountable Care Organisaion
(IACO)?
“Deliver a model for health and care services out of the acute hospital, wrapped around
the patient and co-ordinated by their GP; designed and delivered around local patients
in neighbourhoods. Ultimately delivering one service which is provided by one team,
with one budget.”
(Delivering Together for the people of South Kent Coast/Thanet Integrating health and Social Care, “ One
Service, One team, One Budget”, Compact agreement 2015-2018).
What do People want from Mental Health
services?
 I can access the right help when I am in crisis.
 I can choose what an independent and purposeful
life means to me.
 I am supported to manage my mental health and
physical health together.
 I can plan my care with people who know me and
who work together to do what is best for me.
 I live in a community which has an understanding
and acceptance of mental health.
Integrating Mental Health in Primary Care
Primary Care Team (PCT)
• Mental Health and Dementia
Practitioners will be core members
of the Primary Care Team
• Gateway for Community Mental
Health Services
• Clinical Triage will take place in
Primary Care
• Aligned Consultant Psychiatrists
will provide specialist
consultation, support and
management advice
Secondary Mental Health Care Services (younger and older adults)
• CPA based interventions, case management (including medicines
management for secondary complex and high risk need
• ‘Stepped Up’ and crisis support provided through an integration of CRHT
and CMHT capacity
• Gatekeeping of locality inpatient acute beds
• Access to specialist services (Forensic/eating Disorder/Learning Disability)
Multi- Disciplinary/Multi-Agency
Team
• A ‘virtual’ integrated team
(IAPT/Live Well/primary care
Social Workers/DWP Link
Workers/Turning Point etc)
• Dementia Care services will
work with the Intermediate
and Enablement teams to
provide joint
assessment/treatment and
crisis management
• A ‘walk-in’ hub will provide
same day access for
‘unscheduled’ mental health
and dementia care need
linking to SPoA, the PCT and
Psychiatric Liaison
