Out of State CORI – New Employees must get out of state CORI if they lived or worked
in a state other than Massachusetts.
Three (3) year CORI Cycle for Teachers will begin in January 2012. Please keep track of
who needs to be CORIED;
Please make sure you are using the updated CORI Forms with Kevin Mulvey’s name on
the form. This form can be downloaded from the QPS Website;
All volunteer CORI’s should be tracked using STAR Base under the “Affidavits” entry
tab. Secretaries will be advised of this at the Secretary Team Meeting. Date CORI was
entered and date it expires should be entered. Expiration date is three years from date last
CORI form was submitted.

90 Day period for new teachers – Must act before mid-November
1 Year period for all other employees;
Those with concerns but not willing to terminate at this point must do regular evaluations
in order to support non-renewal in Spring.

Teachers have not renewed licenses that expired over the summer break (Reminder that
150 PDP’s needed). Please see attached list of teachers with expired licenses.
For those applying and waiting for Professional License, file license extension while
waiting for Professional License;
Apply for Professional License at least eight (8) months prior to expiration of
initial/preliminary license.

100% Teacher Licensure;
Insure that all CORI screenings have been complete;
Integrate/Update Seniority List into the MUNIS System;
Integrate Teacher Certifications into MUNIS System;
Complete all PTS and Non-PTS Evaluations by May 1, 2012. Reminder that the goal for
evaluation time table for Non-PTS Staff is Fall, Winter and Spring;
DESE now requires information on teacher evaluations to be sent electronically to their
database. It is therefore essential that all teacher evaluations are completed on time;
New DESE Regulations on Teacher Evaluations implemented by Fall 2013.
Credits that are submitted to Human Resources must have the official college or
university name designation;
Credits that are submitted to Human Resources must have the teacher’s name on the
document; and
Prior Approval of Courses must be obtained from the HR Office.
Maternity Leave and FMLA
a.
Normal Birth – Maximum of 12 weeks of leave, 6 weeks of this leave is paid as long as
there is sick time to cover the leave otherwise all 12weeks are unpaid; and

b.
Cesarean Birth - Maximum of 12 weeks of leave, 8 weeks of this leave is paid as long as
there is sick time to cover the leave otherwise all 12weeks are unpaid.
VII.
Inappropriate Use of Sick Time
a. Sick time can only be used when someone qualifies for sick leave and cannot be used for
vacation or personal leave purposes.
b. Time off prior to or immediately after a school holiday or vacation must be approved by
the HR Office;
c. Leaves of Absences, regardless of length, must be approved by the HR Office.
VIII.
Additional Business
a. Complete evaluations of all non-professional staff including secretaries,
paraprofessionals and custodians by the end of the school year;
b. Jury Duty payments, if any, must be reimbursed to QPS and must show proof of
attendance;
c. Staff Personnel Directory Update/Correction Form will be sent to all the schools. Please
have staff make any corrections to their personnel information (address, phone number,
name changes, etc.) and return to the HR Office no later than November 1, 2011; and
d. Substitute List issues:
i. Mary McDonald will be handling the Sub-list for the HR Office;
ii. An updated list will be provided via email to all principals every 3 to 4 weeks;
iii. Please do not use anyone that is not listed on this list;
iv. If you wish to add a person to the sub list please contact the HR Office Prior to
using this person as a sub; and
v. If you have any concerns about a substitute teacher please call Kevin Mulvey
immediately.
