Official Rules 2013 Bayou Pages Contest

EATEL Directory, LLC dba Sunshine Media’s 2013 Bayou Pages Contest (the “Program”) begins on August 1,
2013 and ends on November 2, 2013 (the “Program Period”). During the 2013 Bayou Pages Contest Period,
one lucky consumer will be selected as the recipient of the prize of $5,000 (Five thousand dollars and no
cents). The winner will be selected on the 99.9 FM KTDY, 97.3 FM The DAWG, and News Talk 96.5 FM subject
to scheduling. No purchase necessary to enter or win.
1. Eligibility: You must be at least 18 years of age or older to enter and a legal resident of the State of
Louisiana. Only individual consumers are eligible to participate in the Giveaway. Employees and family
members of EATEL, Sunshine Media, The Bayou Pages, Vision Communication and any associated and
affiliated businesses are not eligible to participate in the Giveaway. Businesses or commercial consumers are
ineligible to participate in the Giveaway. The Giveaway is void where prohibited by law.
2. How to Enter: Enter by completing the official entry form in the Bayou Pages Directory or online at
www.bayoupages.com/contest.html . Only complete and accurate entries received by 12:00am (CST) on
November 2, 2013 will be eligible for the $5,000.00 (Five thousand dollar and no cents) cash prize drawing.
For an entry to be eligible, you must find all five (5) items (bait and equipment) and have the correct page
numbers on the entry form. Items are only in the Business Classified, and if found in any other sections of this
directory, do not apply to this contest. Eligible consumers may also enter the Program with no purchase
necessary by MAILING or FAXING ENTRIES TO: “CONTEST” The Bayou Pages, 215 Rue Fontaine, Lafayette, LA
70508 Fax (337) 837-5881 Or ENTER ONLINE AT: www.BayouPages.com/contest.html. Limit of one entry per
person per day. Program entries generated by mechanical or automated means will be disqualified. All nonconforming entries are null and void and will be rejected. SUNSHINE MEDIA is not responsible for late, lost,
damaged, misdirected, incorrectly addressed, incomplete, stolen, illegible or postage-due mail and will not
verify receipt of entries. All entries submitted become the sole property of SUNSHINE MEDIA and will not be
returned. Proof of sending will not be deemed proof of receipt. SUNSHINE MEDIA must receive all entries by
November 2, 2013 12:00am (CST). All entries received after the Deadline will be deemed void. No letters,
correspondence, or contact other than the forms of entry listed above will be considered to be properly
submitted entries.
3. Prize and Approximate Retail Value (“ARV”): The winner will receive a monetary prize of $5,000 (Five
thousand dollars and no cents) (“Prize”). No transfer or substitution of Prize is permitted except at SUNSHINE
MEDIA’s sole discretion and then only for a prize of equal or greater value. Any expenses, including but not
limited to taxes, are the sole responsibility of the winner.
The winner will be announced on Radio and notified via telephone. The Prize must be picked up in person by
the Prize winner only at Bayou Pages office in Lafayette, Louisiana, currently located at 215 Rue Fontaine,
Lafayette, LA 70508. Prizewinner must show valid Louisiana identification. SUNSHINE MEDIA is not
responsible for Prize that cannot be delivered or for inaccurate address entry information. 4. Odds of
Winning: Odds of winning any Prize depend on the number of eligible entries received by the entry deadline.
5. Prize Conditions: The Program Prize winner will be notified by telephone. SUNSHINE MEDIA’s inability to
notify a winner may result in disqualification and an alternate winner will be selected. The SUNSHINE MEDIA
“2013 Bayou Pages Contest” Program winner will be required to execute an affidavit of eligibility, a liability
release and a publicity release (collectively, a "Affidavit/Release"). The Affidavit/Releases must be returned
to SUNSHINE MEDIA within the time indicated on the documents. If such documents are not returned within
that time, or if a winner is found to be ineligible, or if winner does not comply with these Official Rules, the
winner will be disqualified and will not receive any prize. A winner’s entry and acceptance of the prize
constitutes permission for SUNSHINE MEDIA to use said winner and winner’s companions’ names,
photographs, likenesses, statements, biographical information, voices, and addresses (city and state) on a
worldwide basis and in all forms of media, in perpetuity, without further authorization or compensation
(except where prohibited by law). Prize is not transferable and may not be exchanged for other prizes.
Winner is solely responsible for the payment of all expenses and taxes associated with the winning of the
Prize. No more than the advertised number of prizes will be awarded.
6. Program Conditions: The Program is subject to all applicable federal, state and local laws. By participating,
entrants agree to be bound by these Official Rules and the decisions of SUNSHINE MEDIA and waive any right
to claim ambiguity in the Program, or these Official Rules. If any provision of these Official Rules is declared to
violate any federal, state or local law, it will be amended to conform to such law and all other provisions will
remain in full force and effect. Winners and entrants also agree to release, discharge, indemnify and hold
harmless SUNSHINE MEDIA, their parent and affiliated companies and all of their officers, employees and
agents from and against any claims, damages or liability due to any injuries, damages or losses to any person
(including death) or property of any kind resulting in whole or in part, directly or indirectly, from acceptance,
possession, misuse or use of any prize orparticipation in any Program-related activity or participation in this
Program. SUNSHINE MEDIA reserves the right to any video, audio, photographic or print material involving
the winner for promotional use without further permission or compensation.
Any person attempting to defraud or in any way tamper with the Program will be ineligible for prizes and may
be prosecuted to the fullest extent of the law. The decisions of SUNSHINE MEDIA with regard to the awarding
of the prize and the interpretation of these rules shall be final. SUNSHINE MEDIA reserves the right to end the
contest earlier than the date listed or to extend the length of the contest.
7. Official Rules: By participating in the Program, entrants agree to be bound by these Official Rules and the
decisions or interpretation of SUNSHINE MEDIA regarding the Official Rules. Any decision or interpretation of
SUNSHINE MEDIA is final and not subject to appeal. Entrants waive any right to claim ambiguity in these
Official Rules. Acknowledging that the Program will be administered in the State of Louisiana, all entrants
agree that the laws of the State of Louisiana will apply exclusively to the Program and that any dispute will
respect to this Program will be resolved in either the federal or state courts located in the State of Louisiana.
