PRODUCER ELIGIBILITY FORM THEATRICAL MOTION PICTURES The Producers Guild of America (“PGA”) will use the information provided below to determine one or both of the following: 1) eligibility of producers to use the Producers Mark (“p.g.a.”) next to their names in the onscreen credits and in advertisements for a particular motion picture, when submitted prior to its release, and/or 2) eligibility of producers to receive producing honors, including those conferred by the PGA, the Academy of Motion Picture Arts and Sciences, the Golden Globes, and the British Academy of Film and Television Arts. The PGA authorizes use of the Producer Mark only after it conducts an accreditation process that is governed by PGA Rules and Procedures, which can be reviewed online at www.producersguildawards.com. Please note that you do not have to be a PGA member to participate in this process. I hereby authorize the PGA to use the information I have provided in accordance with the PGA Rules and Procedures for the purpose of determining whether I am eligible to use the Producers Mark (“p.g.a.”) and/or receive producing honors and awards in connection with my Producer or Produced By credit for the above-identified motion picture. In consideration of this submission, I hereby forever generally and irrevocably release all of the motion picture’s other producers, distributors, and financiers, and each of their respective agents, employees, successors, and assigns (collectively and severally, “Third Parties”), from any and all claims and causes of any nature seeking any remedy whatsoever, at law or in equity, arising from, relating to, or in any way connected with this submission or the use, failure to use, or omission of the Producers Mark (“p.g.a.”). Furthermore, I expressly designate such other producers, distributors, and financiers as third party beneficiaries of this release. I acknowledge the PGA has a proprietary interest in the Producers Mark (“p.g.a.”), which may not be used as a credit designation without the PGA's express written permission, and I agree to be bound by the aforesaid Rules in all other matters arising from or related to my certification for and use of the Producers Mark, and to be subject to the terms and conditions of the agreement, if any, between the PGA and the distributor of this motion picture.
