communities of Prahran, St Kilda, South Melbourne and Port Melbourne. Approximately
270 staff work in multi-disciplinary teams to deliver programs and services across four
key service locations: Southport (in Coventry Street, South Melbourne); Inkerman Street,
St Kilda; Mitford Street, St Kilda and Malvern Road, Prahran. In addition other staff are
located in and provide services in public housing high-rise estates. The Inner South
Community Health Service Ltd. is a registered company managed by a Board of

This role is in partnership with the Inner South East Mental Health Alliance
(ISEMHA), New Horizons Program.
This role will increase the capacity of Moderate Home Based Outreach services and
promote access of clients stepping down from the New Horizons Program. The New
Horizons Program consists of an Intensive Home Based Outreach component, a SECU
Diversion & Substitution component and Care Coordination. ISEMHA, the overarching
consortium sponsor of New Horizons, works to ensure people living with a mental illness
in the inner south east of Melbourne receive appropriate clinical and psychosocial
support services when and where they need them; and that communities and community
services are engaged in creating a sense of connectedness and supporting opportunities
for meaningful consumer participation. The aim of the New Horizons initiative is to
strengthen the capacity of the specialist service system to provide best practice treatment
and support to targeted clients with severe and enduring mental illness and high level
psychiatric disability.

Mental Health Services of ISCHS and also liaise closely with the New Horizons Team
consisting of staff from Inner South Community Health Service, Alfred Psychiatry and
Prahran Mission. The Moderate HBOS Case Manager will also liaise closely with all
partner organizations of ISEMHA to facilitate throughput options for clients of the New
Horizons Program.
3. POSITION DETAILS
The Moderate Home Based Outreach Case Manager works with the New Horizons team
to target clients ready for discharge from that team. The Moderate HBOS Case Manager
will provide Case Management services to appropriate clients exiting the New Horizons
program with preference to clients exiting Intensive HBOS and SECU Diversion. The
Moderate HBOS Case Manager will also work with the New Horizons Program to identify
other appropriate exit pathways for clients.
The Moderate HBOS Case Manager sits within the Mental Health, Alcohol and Drugs
and Counseling Program, as part of the Primary and Mental Health area.
Mental Health and PHaMS provides a range of flexible support options, creative and
recreation group activities, and community integration for people with psychiatric disability
associated with serious mental illness. The Program incorporates PDRSS Home Based
Outreach Services and the PHaMs Program. It is primarily an outreach program,
conducting most of its work in the community, as well as providing some supported
accommodation through our Housing Support Program. The Program promotes and
optimizes strong links with a local network of agencies to provide clients with access to
the widest range of services available.
The Mental Health & PHaMS Teams are staffed by a variety of Mental Health staff, (such
as Registered Psychiatric Nurses, Social Workers, Occupational Therapists and Peer
Support Workers), who work in small teams at Prahran, Southport and St Kilda
Community Health Centres. Most of the service provision is locally based, with some
activities provided across the Inner South region.
3.1
Position Objective
The worker in this position will provide Moderate HBOS Case Management to people
exiting New Horizons. The worker is responsible for providing a range of recovery
focused, goal-oriented, client-focused disability support services and case management
services to individuals in the target group. Another key aspect of the role will be to ensure
that, where possible, clients have coordinated and assisted access to services via
referral and linkage into mainstream and specialist services.

Responsibilities
Client Related Responsibilities

Assess the disability support needs and provide disability support, rehabilitation and
case management services, to persons with psychiatric disability exiting New
Horizons program.

The provision of recovery –focused mental health assessment and intervention
services to the target group

Participate in the development of community work projects and programs designed to
enhance the community’s ability to respond to the needs of the target group.

Maintain accurate client and program records and statistics as required.

Participate in case allocation and case review as required.
Team Related Responsibilities

Provide discipline specific input to the team’s work and to other programs within the
service.

Participate in Case Review and Exit discussions within New Horizons to ensure the
most appropriate exit options for New Horizons clients.

Contribute to the development and maintenance of a supportive environment within
the team.

Participate in specific work unit within the team as directed by the Program
Coordinator or Manager.
Network and Liaison

Liaise and consult with service providers from other ISCHS programs to facilitate
primary care services to the target group.

Liaise with other psychiatric service providers to ensure effective service delivery and
continuity of care for the Program's clients.

Liaise with a range of clinical, community groups and agencies with a view to
improving the provision and co-ordination of a broad range of services to the target
group.

Liaise with all agencies within ISEMHA to support throughput of clients of the New
Horizon’s program.

KEY SELECTION CRITERIA (Must be submitted with your Application)
Mandatory

A professional qualification of either Occupational Therapy, Social Work or
Psychiatric Nursing and appropriate registration/ eligibility for membership in
professional organizations or a relevant health qualification.

Experience working in a mental health setting.

Experience and demonstrated ability to provide recovery focused disability support
and rehabilitation services to persons with psychiatric disability as a result of serious
mental illness.

Demonstrated sound base knowledge in recovery-focused mental health theory and
practice

Ability to work effectively under supervision.

Well developed interpersonal, communication and negotiation skills.

award conditions or SACS Level based on qualifications and
experience.

Full-time negotiable
To meet Client, Program and Service needs, some out-of-hours
and/or weekend work may be required. Any time worked in
excess of the agreed hours per week is expected to be
negotiated with the coordinator and taken as time-in-lieu.

Salary Packaging Information:

Salary packaging is a legitimate method of restructuring existing salary into a
combination of salary and expense payment benefits in order to provide a higher net
remuneration for the employee.

This offer applies while Inner South Community Health Service continues to be
granted "public benevolent institution" status and enjoys exemption from the
provisions of the Fringe Benefits Tax (FBT). Should our status change salary
packaging will no longer be available.

We offer 3 types of Salary Packaging:o
Standard tax free salary packaging amount $16,050 [per annum

Meal Entertainment Expenses $5,000 per annum
o
Entertainment Facility Leasing Expenses - no threshold per annum

It is not compulsory for employees to take up the offer to ‘package’ their salary;
however the benefits from packaging are considerable. There is a small cost to the
employee.

