Bon Secours Health System Awards Ceremony 2016 Outstanding Team Initiative Nomination Guidelines The Outstanding Team Initiative Award will be given to a team whose efforts supported the organisational goals of quality, the patient experience and/ or financial performance. This team must have collaborated towards a common goal and contributed to the success of the organisation. The team must consist of at least three members including the team leader and can consist of members working in the same or different disciplines, e.g. Nurse, Porter, Doctor, HCA etc. A team is comprised of three or more team members, including the Team Leader and can be cross functional or work-unit based. One person on the team must be designated as Team Leader. This could be either clinical or a back office Team e.g. Finance, Bookings or Admin. Marking Criteria Measureable Efficiencies Results Implementation: Improved a process, outcome or focus area within quality, patient experience and/or financial performance. Demonstrated tangible outcomes such as patient satisfaction, cost savings, generation of capital, productivity, quality and/or safety. Can it serve as a model that other departments can implement within the Bon Secours Health System? Requirements and Eligibility The nominee must be part-time or full-time, and actively employed with Bon Secours for at least six months on or before the closing date for nomination. Volunteers are not eligible for this award. Nominees can be any grade/level of staff. Nominees must not have any performance/disciplinary issues within the past year. Nominees must exemplify the Bon Secours mission/vision statements and core values. The nomination must be endorsement the hospital manager Submission Process and Instructions The form must be typed – we cannot accept handwritten forms. Completed forms should be uploaded on the online nomination page. Optional Supplementary materials should be included in the appendix and limited to three pages. The deadline for nominations is 6pm, March 21st 2016. Application Review Process All applications will be reviewed by a Judging Panel, which includes Bon Secours Health System Executive and Non-Executive members and leaders from outside the organization. The shortlist will be announced on the week of the 11th April and the event will take place on May 6th.
