Personsorgroupsmayrequesttheuseofthefacilityformultipledatesoveranextended
periodoftime.Approvalshallbebasedonavailability,takingintoconsiderationanticipated
LEAFusageandotherrentaloptions.
TheBoardofDirectorsofLEAFreservestherighttodenyuseoftheLEAFEducationCenterifthe
proposeduseisinconsistentwithordoesnotpromoteLEAF’smission.

CompletingandSubmittingaFacilityUseApplicationforApproval
Step1.
ThepersonorgroupwishingtousethefacilitymustcontactLEAFtodetermine
theavailabilityofthefacilityontherequesteddate(s).
Step2.
Iftherequesteddate(s)isavailableandapprovedbytheadministrator,thenthe
followingmustbesubmittedtotheLEAFofficeat1501W.6thStreet,Erie,PA
16505:
ü CompletedFacilityUseApplication
ü IndemnificationForm
ü CertificateofLiabilityInsuranceineffectforthedate(s)ofusewith
$1,000,000foreachoccurrenceand$2,000,000generalaggregate
(CategoryIIonly)
ü 50%ofrentalamountasanon-refundabledeposit.Checksshouldbe
payabletoLEAF.
Step3.
Therequesteddatewillbeheldfor7daysfromthedateofinquiry.Ifthe
requiredpaperworkanddepositarenotreceivedwithin7days,thefacilitywill
bemadeavailabletootherswithnoobligationbyLEAFtocontactthepartythat
madetheinitialinquiry.
Step4.
IfapprovedbyLEAF,thesignedoriginalFacilityUseApplicationwillberetained
byLEAFwithacopyoftheFacilityUseApplicationprovidedtothepersonor
groupusingthefacility.
Step5.
Anyoutstandingbalanceisdue2weekspriortotheusagedate.
Step6.
Afewdayspriortotherentaldate,thefacilityadministratorwillprovidethe
renterwithbuildingentryinstructionsandotherpertinentinformation.
Step7.
Arefundoftherefundableportionoftherentalwillbegivenifpaidand
cancellationismadepriorto2weeksbeforetherentaldate.Norefundwillbe
givenforcancellationsmadewithin2weeksoftherentaldate.

FACILITYUSERULES&REGULATIONS
1.
No group or person contracting to rent the LEAF Education Center shall assign
his/her/theirapproveduseofthefacilitytoanothergrouporperson.
2.
Any group or person using the LEAF Education Center shall be responsible for
supervising the activity to assure that it begins and ends in accordance with the
approvedcontractandtoassurethatparticipantsintheactivityconductthemselvesina
propermanner.
3.
Any group or person using the LEAF Education Center shall leave the premises in the
mannerinwhichtheyfoundit.Tablesandchairsshouldbeneatlystacked,thecounter
top wiped, and the floor swept. Trash should be placed in the green barrels in the
parkinglot.Broomsarelocatedinthesupplyclosetandextratrashbagsareunderthe
sinkorinthesupplycloset.Donotreplacethetrashbagsintherestrooms,asakeyis
requiredtochangethebags.Alldoorsandwindowsshouldbeshutandalllightsturned
offpriortoleavingthepremises.
4.
Any group or person using any facility of LEAF shall be responsible for any and all
damages which happen during the time when the building/grounds are being used by
the group/person will be billed for such damages to make payment to LEAF for these
damages within ten (10) days of the billing date. It is also the responsibility of the
group/person renting said facilities/grounds to file the claim(s), etc., with their
insurancecompanyiftheorganizationhassaidcoverage.
5.
In accordance with state and local laws and LEAF policy, smoking, tobacco products,
illegaldrugsandlook-alikedrugsarestrictlyprohibitedfromallbuildings,propertyand
grounds. Smoking is prohibited in the LEAF Education Center. Alcoholic beverages are
permittedwithpermissionofLEAF.
