Introduction
During your Placement you will be expected to complete a care plan. Details of the expected
content are provided in this pack.
The care plan will contribute to the assessment of the following learning outcomes:LO1, 2, 4, 5, 6, 7, 8 & 9
Guidelines
 The care plan is an opportunity to fully review the care of one patient in any setting it is not
intended to be a lengthy theoretical document
 The aim of the care plan is for you to apply theory to practice and to use knowledge and
clinical reasoning to demonstrate understanding of all elements of the care of a patient
 The care plan should be appropriate to dietetic practice and to that end the following
methods of presentation can apply:

Present orally to a small group of dietitians

Present/discuss at a clinical supervision session

Discuss at an MDT
-
Present/discuss at a ward round
-
Oral discussion via a teaching clinic
(your supervisor will guide as to how the care plan should be presented)

The care plan presentation should be approximately 10 minutes in length

Supervisors assessing the care plan should receive your completed care plan report prior
to the presentation so your supervisors can make notes for discussion

The care plan report should be no more than 4 sides of A4

Action points to improve skills/ knowledge should be agreed between supervisor and
student at feedback/feed forward meeting
Selection of a an appropriate patient
Your supervising dietitians will guide you in the selection of a suitable patient and to ensure
that the patient has consented to being used for the purposes of this study.
Where possible, you should try to select a patient with whom you are involved in their
dietetic care to ensure that this work remains a practical exercise. This will allow your skills
and abilities to respond to changing situations to be assessed.

Good practice tips –student
1. Any acronyms for example NG, PEG should only be used if they have been previously
defined
2. Always report units of measurement
3. Critically justify your work . Do not just report information, show that you understand what it
means and can apply it to your patient
4. This is also an exercise in professional documentation. Use professional language and
terminology at all times e.g. ‘feeder’/ monitor ‘bloods’ are inappropriate terms
5. Report accurately and specifically , i.e. for biochemistry ‘ below normal’ , ‘low’ are not
acceptable descriptors
Assessor Tips

Assessors should ask questions following the presentation to clarify points in the care plan.
This will provide students with the opportunity to show what they have learnt or to review
areas that remain unclear .Please note on the care plan report what areas are to be
queried

The assessors should write notes on the care plan report to facilitate learning and should
together with the student produce a number of action points

Please ensure that the care plans are scrutinised and students are carefully questioned to
facilitate a full and clear understanding on the part of the student.
