Not Administered at Select this to record an immunization administered at another facility for which you your facility have information in addition to vaccine and vaccination date. Does not require identifying the other facility. Constrain entry to  Check this box if the facility uses the USIIS Vaccine Inventory module; it helps vaccines in prevent data entry errors that affect inventory counts. inventory  Check this box to require vaccine information to be driven by Lot Number, selected from the Unit of Use Lot Number pick‐list.  After selected, the Unit of Use Lot Number, Vaccine Code and Description, and Manufacturer Code and Name automatically fills in—they cannot be entered separately.  The box cannot be unchecked for users who have been configured by clinic manager(s) for this feature. 

The box cannot be unchecked for users who have been configured by clinic manager(s) for this feature. Uncheck this box to allow data entry of lot number, vaccine code and manufacturer independently.  Lot Number may be manually typed into the field.  Required.  Double‐click in the data entry field to automatically fill in the current date.  Required.  Entry determines what selections display in the Unit of Use Lot Number pick‐list. For example, if VFC Medicaid is selected for Financial Class, then only Unit of Use Lot Numbers set up in inventory as VFC funded will display for selection.  Entry also determines what Inventory Borrowed From options display for selection. Inventory Borrowed  Use to record if the vaccine used for the immunization was from stock not From matching the patient’s eligibility entered in Financial Class.  Pick‐list selections are determined from the Financial Class entry.  For example, if Financial Class is Non‐VFC Eligible, then Inventory Inventory Borrowed From selections will include VFC and Special Projects—not Private.  Reason is required.  If entered, Inventory Borrowed From funding category determines what Lot Numbers display for selection in the Unit of Use Lot Number pick‐list.  For example, if Inventory Borrowed From is VFC, only VFC inventory will display in the Unit of Use Lot Number pick‐list. Page 2 of 4 Unit of Use Lot Number  If entry is “Constrained to vaccines in inventory”, entry must be from selections in pick‐list—which is based on Financial Class.  If entry is not constrained to vaccines in inventory, entry may be manually entered or selected from the Unit of Use Lot Number pick‐list—which is based on Financial Class. Deduct from  Display only – not a data entry field.  Displays if “Constrain entry to vaccines in inventory” is checked.  Indicates which inventory type will be decremented when the immunization is saved. Vaccine Brand  Displays if entry is not constrained to vaccines in inventory.  Selection of Vaccine Brand will automatically fill in Vaccine Brand, Vaccine, and Manufacturer. Vaccine Code  Required.  Will fill in automatically if Vaccine Name is entered.  Can be data entered manually if ‘Constrain entry…’ box is not checked. Cannot be a value greater than 999.99. Pick‐list selections are based on CDC VIS published dates for the Vaccine. Double‐click in the data entry field to automatically fill in the current date. Enter the name of the person who administered the immunization, first name followed by last name. Immunization Route, options from pick‐list. Immunization Site, options from pick‐list. Optional. As applicable, adverse reaction following immunization. Select from pick‐list. Optional. As applicable, reason why vaccine administered was not valid. Pick‐list options include: Manufacture recall, Stored too cold, Stored too warm, Other. Immunizations Screen Menu History Displays the Immunizations History screen with the patient’s immunization history. Enter Detailed  Displays the Immunization Detailed Entry screen.  Use to enter administered immunizations.  Use to enter non‐administered historical immunizations when have information in addition to the vaccine code and vaccination date. Enter Brief  Displays the Immunization Brief Entry screen.  Use to enter non‐administered historical immunizations.  Do not use to enter administered immunizations. When select entry, Vaccine Code automatically fills in.  Fills in automatically when enter Vaccine Code. Vaccination date ‐ required. Date  Fills in automatically when select Vaccine Name from pick‐list.  Can enter code directly. If enter directly, Vaccine Name automatically fills in.
